Because: the metaphysics he currently accepts is that he's a piece of trash for cheating and that he deserves to die. His echo chamber puts all of them blame on him and he is not engaged with any community that would tell him that being in a government contract marriage without sex IS miserable. He considers it a failure of character that he wanted the sex and that the sex wanting lead to cheating. It's partly right, but it does not acknowledge that he was doomed to fail. Also, that Tamara vid where pics of Hila are burning and Ethan looks dead inside seem to imply that she wasn't just a lady in love but rather that she was intent on destroying his marriage for reasons beyond attraction. I think he felt fooled.
