*Video and images shown with full, written informed consent from the patient. Our first ESD of 2024, performed last week, using our saline-immersion technique (SITE)-facilitated, tri-tunnel variation of the pocket-creation method (PCM). The lesion was a 12 x 9cm laterally spreading tumour involving the ileocaecal valve, caecum and ascending colon*. Dissection speed: 28.3 mm^2/min. SITE, which we first described in 2016, enhances patient comfort (avoiding the need for deep sedation), obviates the need for any traction (through buoyancy), enhances optical clarity (through elimination of dissection-related smoke, fat micelles, debris and through intrinsic magnification), and enhances visualisation of vessels and bleeding points. The use of saline (not water) is critical for conductivity. High-effect, forced coagulation enables very effective knife-alone management of vessels and any bleeding, and this, coupled with the use of self-assembling protein matrix gel (Purastat), obviates the need for the use of coagulation forceps. All these factors contribute to increased efficiency, effectiveness and patient comfort. Histopathology of the specimen confirmed complete (R0) resection and showed that the lesion consisted of a low grade dysplastic lesion without any evidence of invasive malignancy, confirming cure. ESD is an endoscopic microsurgical technique which affords an en bloc, organ-preserving, minimally invasive cure for such lesions, which unlike piecemeal resection, offers R0 with virtually no risk of recurrence; this latter point eliminates the need for tight surveillance and related patient inconvenience/risk, also reducing cost and environmental impact. Onwards and upwards for the betterment of minimally invasive care for all our patients!

In this operative video, Aaron Cohen-Gadol, MD demonstrates the contralateral cross-court approach for clip ligation of a right P1 aneurysm through a left-sided pterional craniotomy. The basilar artery, its bifurcation, and the contralateral P1 segment are exposed, revealing a perforating vessel arising from the neck of the P1 aneurysm. The cross-court approach is effective in this case, as the anatomy provides favorable visualization for circumferential dissection of the neck. A temporary clip is placed on the basilar trunk to allow controlled manipulation of the neck for definitive clip application. The narrow working space medial to the carotid artery is used to inspect the perforating vessels. From compliance to working safely around humans to whole-body manipulation of heavy objects, combining force and position control can dramatically expand the capabilities of robots. This is especially true for legged robots, which have so much ability to exert forces on the world around them. But how do we train robots which can do this? At this station, students dive into a variety of hands-on activities designed to reinforce their understanding of the letter I. The Nordic Trio: A Tough Match for Russian Corvettes Russian corvettes are among the most heavily armed on the planet, but they would run into trouble against their Nordic counterparts. The Russian corvettes of the Karakurt class are the most heavily armed ones I know of. They carry Kalibr missiles with a range of 2,500 km and hypersonic Zircon missiles. They would be capable of inflicting significant damage on the entire European fleet, which lacks defenses against the Zircon. The Karakurt also features a version of the Pantsir system with a range of 40 km, tested for intercepting ballistic missiles. Yes, the Karakurt demonstrates how the concept of sea dominance can shift with even small vessels capable of projecting power over long distances. However, there is a trio that would serve as a formidable barrier to the Russian corvettes: the Skjold, Hamina, and Visby. The stealth corvettes from Finland, Sweden, and Norway would be a massive headache for the Karakurt, precisely because of their small size and stealth architecture. The Karakurt may possess Kalibr missiles with a 2,500 km range or Zircons capable of reaching nearly 1,000 km, but to locate their targets, the Karakurt relies on drones like the Altius-U, equipped with a multimode sensor suite and multiple capabilities. Facing this trio would not be an easy match for the Altius, precisely because these vessels are designed for operations in contested environments, with an emphasis on low observability. Imagine trying to locate the Skjold, with its advanced stealth design featuring radar-absorbent materials, angular shapes, and a drastically reduced RCS, weighing only 270 tons? On the Altius's radar, it might appear as a small fishing boat under good conditions. And what about the Hamina? Built with aluminum and a composite superstructure (Kevlar and absorbent materials), it reduces radar, thermal, magnetic, and acoustic signatures. With a very low RCS, it would blend into maritime clutter (waves and small objects), operating in silent mode with electronic jamming. And the Visby? This is the stealthiest of all. Made from carbon fiber composites, with angular shapes, retractable/embedded weapons, and reductions in all signatures (RCS up to 99% lower, IR, acoustic, magnetic, hydrodynamic wake), plus advanced jamming and optimized operations to deny aerial sensors. This leads to less effective deformation with each blow, requiring more strikes before the workpiece cools, which can result in incomplete shaping, uneven grain flow, surface defects, or even cracks from rushed or insufficient hammering. Precision also suffers because vibrations make it harder to land accurate, controlled blows, especially during detailed operations like drawing tapers, punching, or fullering. fable, I want you to grab a MetaHuman file off of Unreal Engine Marketplace or Fab Marketplace or something like that. There should be free ones. Find a character controller that works with that for web and threeJS. Specifically for flying, like Superman, generate a World Labs outdoor environment of your choosing and set up a threeJS project where we can fly around with a character like Superman, with really nice flying animations, camera animations, and camera zoom and shake effects when you're flying. Put some nice post-processing on it with some bloom and grain and stuff like that. Really just make this feel as polished as you can, where we get to fly around in a world where the flight feels similar to Superman. Do it in one shot and without any input from me. optimize it 2 prompts - NOT BAD!! Honestly could get this feeling/looking 10x better with probably 2 more prompts. I'd likely use a larger city GLB, populate the outskirts with World Labs gsplats, manually grab a better character to use, and add some objectives. Can CGMs be harmful? Continuous glucose monitors are big business these days, and for sure, they can be helpful. Especially for people with type 1 diabetes and anyone treated with insulin, they can be a godsend. 🙌 However, they are now also promoted as tools for anyone to "avoid blood sugar spikes" to improve health and weight loss. This obviously expands the market massively, but is this a good and helpful way to use them? I would argue a CGM can be a good tool, if you know how to use it right. However, there are a couple of concerning risks if you don't, as I discuss in this clip: 1. Adding fats on top of any food will lower your blood glucose spike, but it can also add tons of extra empty calories, which may be counterproductive for weight loss, etc. In effect, lowering your blood glucose spike this way may lead to weight gain, not weight loss. 2. Healthy people on strict low-carb diets tend to develop "Adaptive Glucose Sparing," a fancy way of saying their glucose levels become very sensitive to even small amounts of carbohydrates. Wearing a CGM in this situation may make people afraid of even tiny amounts of carbs, like a few blueberries, thinking that any carbs are dangerous. A couple days of eating slightly more carbs could remove this issue. Berries and similar foods are hardly dangerous for healthy people. With those two major caveats, I'm all for testing CGMs and learning about blood glucose reactions to food. What is your experience with CGMs? Please share in the comments.
