A... surgeon at Winnipeg’s Health Sciences Centre removed dead tissue from her knee, then sent her to Concordia Hospital with the plan to bring her back that same day so a specialist could stitch the wound (CBC News). On a Friday in December, Roseanne Milburn lost her 𝐫𝐢𝐠𝐡𝐭 𝐥𝐞𝐠 — over a missing hospital bed. This is not a freak accident. It is the predictable output of a system that rations care by making people wait. In 2025, the median Canadian waited 𝟐𝟖.𝟔 𝐰𝐞𝐞𝐤𝐬 from a GP referral to actual treatment (Fraser Institute). For orthopedic surgery — the exact category Milburn needed — the median wait is 𝟒𝟖.𝟔 𝐰𝐞𝐞𝐤𝐬. Nearly a full year. By design. That is 222 percent longer than the 9.3-week wait Canadians faced in 1993 (Fraser Institute). The system isn’t getting better. It’s getting slower — and the waiting list itself becomes the rationing mechanism. Defenders call it “𝘧𝘳𝘦𝘦”. It is not free. Roseanne Milburn paid for it. She paid with her leg. Every day hear stories about people not getting care that they deserve to have. Our system is f’d. my friend went for open heart surgery was released in two days. He went home and was suffering with fever pain and low blood pressure. Rushed to hospital and had his large intestine removed and is on s colostomy bag for life. He’s in IcU and gravely ill. Alberta health care is failing.

My mother in law, 81 and feisty, went into a Calgary hospital for a persistent back pain. She was dead after two weeks. They blamed it on an antibiotic-resistant infection which she didn't even have when she went in. Apparently rampant in that hospital. Tubes, injections, all kinds of weird meds. It was horrible to watch her go down so fast, and in a hospital, too.

Now she could not walk into a room without being helped, her body showed the signs of torture wherever they have kept her since they took her. Days missing, her family never told where she was. Then she is brought to court like this and a sane magistrate accepts the charges against her and sends her to prison. She begged. She cried in that courtroom, asking the magistrate for bail so she could simply get to a hospital and have a scan, just to know whether something inside her was broken. She said, plainly, that she would not survive a night in prison without help. She opened it, went to the transcript, and copied the whole thing. She pasted that transcript into Claude and asked it to turn the format into a prompt for a similar video. Claude wrote the prompt. She pasted the prompt into an AI video generator. A few minutes later she had a finished cartoon. Bright colors, dancing animals, big letters, cheerful music. Indistinguishable from the one she copied. She posted it. Then she did the exact same thing the next day. And the day after. And every single day for 30 days. She never wrote a script, animated a frame or recorded a single sound. She found something that already worked, fed it to Claude, let an AI rebuild it, and uploaded the result before she went to bed. Nobody knew who she was. No name, no face, no personality, no story. Just a channel quietly posting toddler cartoons every day. And YouTube paid her anyway, because YouTube does not pay you for being interesting. It pays you for watch time, and toddlers watch the same video until the battery dies. By the end of the month the payments were real. $20,000 to a girl who will tell you to your face that she is not smart. That is the part nobody wants to sit with. The barrier was never intelligence. FMC JABI, ABUJA, A HUGE MESS OF A HOSPITAL The Federal Medical Center (FMC), Jabi, is supposed to be the best government hospital in the FCT, but it is today a huge mess. Some years ago, my wife was ill and was admitted to that hospital, and I did a post about my experiences there then. It was at least ok. Four years ago, we had our son at that same hospital. My experience weren't that satisfactory, but at least, if you look at Nigeria, it was also ok. A lot of the staff were very badly behaved. We paid for a private ward but didn't get most of the services that came with it. I had to run around the hospital for one week myself even when we were supposed to get someone do all that for us as part of the package. But in all, I wasn't bothered because I have my son. Over the years, I have watched that hospital deteriorate from 'BAD to WORSE'. For a start, the compound is so small. Those who planned it didn't plan with the population of Nigeria in mind, something very common in our country. Recently, they have been squeezing structures in every corner that the hospital is now shocking. But this is not even the issue. On Monday, 14 May, we rushed a family member who is sickle cell to the FMC. This was at about 1:PM on that day. For a sickle cell patient who was in crisis and crying of pains, the FMC didn't have a bed for her at the emergency ward. They were giving her treatment, including drips, while she was sitted on a chair for hours and crying of pains. At a point, they went to create a bed space for her, only for them to tell us that another patient who obviously has also been waiting took the bed before we knew it. The services was zero, the environment, nothing to write home about. Professionalism from staff? Forgerrit. We waited for hours, and in the process, another family friend told my wife about SAHAD HOSPITAL, right inside Apo Resettlement. The same person who owned Sahad Stores owns the hospital. I have never heard of it before. My wife demanded she should be discharged. They tried to discourage it, but she insisted. She was asked to sign, which she did. We drove to Sahad Hospital, and I was really impressed. The whole environment is very neat and clean. A big hospital with a lot of parking spaces. It didn't end there. While it cost 13k to register at the FMC, which is a government hospital, it cost just 3k to register at Sahad. She was put in a bed immediately with AC and even ceiling fans working. They attended to her immediately. Their services are quite cheaper and better than the FMC. Of course, there are also a couple of issues with staff conduct, but the FMC is also worse in that regard. She was transferred to the female ward on Tuesday, and it is still the same thing, clean beds, clean rooms, 24/7 electricity, fan, and AC. In all these, i wasn't surprised. A country where the minister of health claims his ministry only got N36m so far from the 2025 budget in 2026, such shouldn't bother you. The country is sinking into debt by the day, yet all the infrastructures are decaying. They don't care about the people, so they can't care about the hospitals they and their families don't use. Another peer-to-peer call. The reviewer was kind. Really kind. But, respectfully, not my peer. He listened as I explained why my patient needs lymphovenous bypass surgery. It’s a surgery that can treat the painful swelling in her arm caused by breast cancer treatment and prevent it from getting worse. She has done everything right. Compression. Therapy. Expensive garments. She’s changed her life to spend an hour a day with every resource she can afford. It’s affecting her ability to work as a nurse. And she is getting worse. This is the window where we can fix it. Early enough that surgery can actually change the course. We can do this surgery outpatient. Two hours. CMS even has a code for it. The man on the phone agreed it would help her… But he couldn’t approve it… Because he’s not a plastic surgeon and doesn’t have the specific knowledge necessary to make this recommendation. He gave me the help he felt he could. He told me to appeal and told me not to expedite it, because that wouldn’t work in my favor. Every human involved in this case knows what we should do. But the system is inhumane.

My last case of 2025 and my first case of 2026 tell you everything you need to know about our healthcare system. Last week, I was scheduled to do a DIEP flap for a patient who had been waiting a long time. We had asked her insurance company if we could do the surgery in network at @Redbudsurgerycenter, where we have everything needed to safely and efficiently take care of her. They hadn’t approved that, so the surgery had to be scheduled at an older hospital in town. When we arrived, the hospital was having HVAC issues. The humidity levels were not safe for surgery. We waited for hours, and ultimately the case had to be canceled. It wasn’t just her surgery. Multiple surgeries were canceled that day because operating under those conditions would have been a fire risk. We weren’t able to reschedule her surgery before the end of the year, which means her deductible resets. I could have safely done her surgery at Redbud that day but insurance rules wouldn’t allow it. This is what closed networks and rigid systems look like in real life. Now here’s the contrast. The first case I’m doing in 2026 is also a woman who needs breast reconstruction with DIEP flaps. This time, instead of traditional insurance, her employer uses a third party administrator. They came to me directly and asked what I would charge if they paid fairly and transparently. On Monday, we’re doing her bilateral DIEP flap at @Redbudsurgerycenter. I can control the environment, the costs, and the care. It’s better for the patient and significantly less expensive for the employer. This is called Direct Specialty Care. It’s not direct primary care. It’s specialty care delivered without unnecessary barriers. It’s happening. We’re starting the year this way. And I’m going to keep pushing the boundaries and keeping you informed.
