Paste your current passport-size photograph 3. If your name is in the 2002 Electoral Roll: fill in your Elector details — Name, EPIC No., Father's/Mother's Name, District, State, Assembly Constituency (AC) Name, Assembly Constituency (AC) Number, Part No. & Serial No. 4. If not, but a parent's/grandparent's name is: fill in their details instead in the Relative's column 5. Enter your current Date of Birth, Aadhaar Number (optional) & Mobile Number 6. Add Father's/Guardian's, Mother's, and Spouse's (if applicable) Name & EPIC Number 7. Sign, or affix your left thumb impression 8. Return the form to your BLO for verification and countersignature 9. The form is also available for download at 10. Your name is added to the draft roll only after the BLO receives your completed form 𝐈𝐦𝐩𝐨𝐫𝐭𝐚𝐧𝐭 𝐭𝐨 𝐫𝐞𝐦𝐞𝐦𝐛𝐞𝐫 1. Aadhaar is optional 2.
