The value of Transvesical-assisted cystic pedicle dissection. A large stone appearing at ? Hartman Pouch, ? GB neck. 1- never dissect proximal to stone 2- never assume it is in HP 3- immediately suspect Mirizzi 4- accessing the stone through the GB will help either transvesical removal or dislodgement and facilitate extraction 5- Mirizzi 2 becomes obvious 6- NEVER MIND THE CVS, you risk major duct injury. Transfistula IOC and exploration will be mandatory to guide further dissection. 7- choledochoscopy aids further anatomical confirmation. Seeing the scope light in the hepatocystic triangle while the scope is in the RHD , and with no irrigation fluid escaping duct system slows down the surgeons pulse and stems his/her sweat > the ducts are intact. 8- extract any further stones and insert a transfistula t-tube. No sutures needed but obviously a good subhepatic drain until no bile is seen, t tube remains for four weeks to allow healing of the fistula. Avoid early postop cholangiography ( it can cause contrast and bile leakage with potential pain/peritoneal irritation which slows down the healing and may force reintervention.

My Mouse P.I. For Hire Review: “This game is well crafted, highly engaging as a first person shooter, stuffed with content, and is well suited to be Indie game of the year”. 🟢Play as Jack Pepper (voiced by Troy Baker) as a Private investigator in a town called “Mouseburg.” 🔵Tasked with numerous investigations consisting of main quests, side quests, collectibles, and finding cards for a card game. 🟢$29.99 is a great price point for everything you get to do in this over 15 hour main game. Completion is well over 20 hours. 🔵Multiple weapons to unlock, upgrade, and choose from as you move through the game. Explosives are also unlocked. 🟢The black and white setting is a cool visual twist that immerses us in feeling like an old school PI. 🔵There is a top down map that you can navigate to new unlocked points on a map, think Tiny Tina’s Wonderland. 🟢Multiple graphical options for performance at 60fps and quality at 30fps. 60 was consistent on performance mode. 🔵The musical score of jazz aids to the setting of being an old school detective. 🟢3 difficulty settings, I recommend Detective or Supersleuth as rookie is too easy. 🔵Game encourages finding secret paths off the main sections to locate secrets which might be loot, newspapers, etc. 🟢The crime wall is a cool feature we use to post evidence as we collect items within missions. 🔴Had some pop ins 2 or 3 times on performance mode. They are minimal, but worth noting. 🔴Enemy AI is not smart on lower level difficulty, you need to play on Detective, but I recommend supersleuth to have more enjoyable combat sequences. 🔴Minimal accessibility options to engage with. 🔴Would have liked to see more enemy variety. Game is a 9.1/10 for me. Highly recommend buying!

Mohammad Bagher Ghalibaf, the Iranian parliamentary speaker who led the first round of negotiations with JD Vance in Islamabad, gave an important TV interview today. In it, he lays out his narrative of the war, the diplomatic track, and signals where Iran’s political goalposts are currently. He projects firmness throughout. Iran, in his telling, will not back down from its negotiating demands. At the same time, he frames negotiations not as a retreat from conflict but as its continuation by other means, a way to consolidate gains and secure what he calls a “lasting peace.” 🔴His core framework is that military power, public mobilization, and diplomacy are inseparable: ➡️“Today, we have the ‘field,’ the ‘street,’ and ‘diplomacy of authority.’ These are not three separate things. Do not make a mistake. Today the street, the field, and diplomacy are together.” 🔴He argues this is precisely why now is the moment for diplomacy. Iran, he says, has reached a peak position militarily and in terms of social cohesion, and negotiations are meant to lock that in politically: ➡️“In the military sphere, we are at a maximum point. The street has given strength to the field. We have achieved part of our demands militarily, but they must now be stabilized and recorded politically. Rolled credits on Yakuza Kiwami 3 & Dark Ties: Review: A massive visual and technical overhaul that brings the sunny beaches of Okinawa to life. The new combat system and Morning Glory minigame are the stars of the show, the latter of which has been transformed into a surprisingly addictive farming sim. Kiwami 3 starts out with Kiryu and Haruka leaving Kamurocho to take over the Morning Glory orphanage in Okinawa. Kiryu grew up in an orphanage, so it's only right that he gives back. The early hours of the game are spent introducing you to the new characters, and building bonds with them through the expansive new minigame. Morning Glory is a crucial location to Kiryu's story, and the new storyline gives it impact, rather than just being a backdrop. The pacing is kind of slow in the beginning, but the payoff is a high-stakes thriller that puts both the orphanage and the Tojo Clan at risk. It quickly goes from a slice of life to a CIA thriller. If you've played the original, then some of the character redesigns lack the same charm IMO. Rikiya is supposed to be clumsy and a bit of a goofball, but extremely loyal, and eager for Kiryu's approval. The new character model (although he was great in Tokyo Vice) feels more 'serious' and loses some of the original's energy. Similar can be said about old man Nakahara. Combat has been overhauled however. Dragon of Dojima Style feels refined and heavy, and is probably the best implementation of the classic 'Dragon Style' combat system yet, ideal for taking down bosses 1v1. The new Ryukyu Style is built for taking down mobs. It lets you cycle through an arsenal of weapons, each with unique special effects, and large AoE attacks that can take out entire groups in seconds. In addition to Morning Glory, The "Haisai Girls" minigame is a surprisingly deep minigame that essentially turns this into Dynasty Warriors! You manage a biker gang, recruit members, and fight massive real-time battles against dozens of enemies at once. It has an entire storyline, and I'm surprised it wasn't marketed more! As for Dark Ties, it's a roughly 3 hour playthrough that's a must play for Yakuza fans. Rolled credits on Yakuza Kiwami 3 & Dark Ties: Review: A massive visual and technical overhaul that brings the sunny beaches of Okinawa to life. The new combat system and Morning Glory minigame are the stars of the show, the latter of which has been transformed into a surprisingly addictive farming sim. Kiwami 3 starts out with Kiryu and Haruka leaving Kamurocho to take over the Morning Glory orphanage in Okinawa. Kiryu grew up in an orphanage, so it's only right that he gives back. The early hours of the game are spent introducing you to the new characters, and building bonds with them through the expansive new minigame. Morning Glory is a crucial location to Kiryu's story, and the new storyline gives it impact, rather than just being a backdrop. The pacing is kind of slow in the beginning, but the payoff is a high-stakes thriller that puts both the orphanage and the Tojo Clan at risk. It quickly goes from a slice of life to a CIA thriller. If you've played the original, then some of the character redesigns lack the same charm IMO. Rikiya is supposed to be clumsy and a bit of a goofball, but extremely loyal, and eager for Kiryu's approval. The new character model (although he was great in Tokyo Vice) feels more 'serious' and loses some of the original's energy. Similar can be said about old man Nakahara. Combat has been overhauled however. Dragon of Dojima Style feels refined and heavy, and is probably the best implementation of the classic 'Dragon Style' combat system yet, ideal for taking down bosses 1v1. The new Ryukyu Style is built for taking down mobs. It lets you cycle through an arsenal of weapons, each with unique special effects, and large AoE attacks that can take out entire groups in seconds. In addition to Morning Glory, The "Haisai Girls" minigame is a surprisingly deep minigame that essentially turns this into Dynasty Warriors! You manage a biker gang, recruit members, and fight massive real-time battles against dozens of enemies at once. It has an entire storyline, and I'm surprised it wasn't marketed more! As for Dark Ties, it's a roughly 3 hour playthrough that's a must play for Yakuza fans. $I nearly 2x'd the speed while only using +1GB VRAM with the new MTP update in llama.cpp 🤯 You need to add these flags to start using it: --spec-type draft-mtp \ --spec-draft-p-min 0.75 \ --spec-draft-n-max 2 My results with Qwen3.6 27B on a single RTX 5080 ↓ ⚪️ no flag (without mtp) → 54.3 tok/s with 13.26GB VRAM 🔵 --spec-draft-n-max 2 → 90.7 tok/s with 14.29GB VRAM 🔴 --spec-draft-n-max 2 --spec-draft-p-min 0.75 → 93.9 tok/s with 14.30GB VRAM 🟢 --spec-draft-n-max 6 --spec-draft-p-min 0.75 → 93.9 tok/s with 14.87GB VRAM Increasing to 6 draft tokens didn't help my setup for some reason. I made sure to test with a low context length to have enough headroom and eliminate risk of vram stress. From my understanding: 1) The speed gains are very task-dependent. You need to test across a wide range of tasks to get a realistic idea of the benefits 2) We’re already running heavily quantized GGUF models (Q3, Q4, Q6, etc.), so we already benefit from strong speed/performance thanks to the reduced size. That’s why some people are seeing little to no improvement compared to MLX or other quantized versions The progress over the past few days has been insane to say the least. However, MTP now consumes significantly more VRAM. Personally 16GB just isn't enough to use MTP and run it with a good context size. Time to upgrade lads, 24GB+ users are eating GOOD today 🔥 Full setup below ↓$
