A rare case of sirenomelia or better known as Mermaid syndrome! This is an extremely rare congenital developmental disorder characterized by anomalies of the lower spine and the lower limbs. Affected infants are born with partial or complete fusion of the legs, giving them the appearance of a mermaid’s tail. They may have one foot, no feet or both feet, which may be rotated externally. It presents with a spectrum of anomalies which are incompatible with life: limb fusion, sacral agenesis, gastrointestinal malformations (imperforate anus, rectal atresia), genitourinary defects (bladder exstrophy, absent bladder, ambiguous genitalia), and spinal abnormalities. These congenital defects are fatal and incompatible with life. They lead to complications such as chronic renal insufficiency due to bilateral renal agenesis or dysgenesis, respiratory failure from pulmonary hypoplasia, increased susceptibility to infections due to compromised urinary and gastrointestinal tracts, nutritional deficiencies, and potential cardiovascular abnormalities. These are major causes of perinatal mortality. The exact cause of sirenomelia is unknown and heavily debated, but it is believed that it results from a failure of normal vascular supply from the lower aorta in utero, causing severe ischemia to the caudal portion of the fetus. A commonly cited theory is the “vascular steal” hypothesis, which suggests that an abnormal single umbilical artery diverts blood flow away from the caudal structures, leading to underdevelopment of these areas. Unfortunately, the prognosis for infants with sirenomelia is exceedingly poor, with a high rate of stillbirths and neonatal deaths. The combination of renal failure and severe respiratory distress typically results in death within the first few days of life. Survival beyond the neonatal period is rare and generally requires aggressive surgical and supportive interventions, which often still yield limited success. Early prenatal diagnosis via ultrasound is essential for planning perinatal care, but despite advanced medical interventions, the overall outlook remains dire.

A 34-year-old man is admitted to the emergency room after falling from a motorcycle at high speed. According to witnesses, the patient collided directly with the vehicle’s fuel tank before being thrown to the ground. Upon arrival, he presents with intense pain in the perineal region, progressive penoscrotal edema, and inability to urinate. During the initial assessment, urethrorrhagia is noted from the urethral meatus. On physical examination, an extensive “butterfly wing” perineal hematoma is observed, along with marked tenderness to palpation of the bulbar urethra and slight suprapubic distension. Vital signs remain stable. Would you insert a urinary catheter in this patient? What procedure is being performed on the image? What is the diagnosis?

Most people think surgical glue replaces stitches. In reality, many surgeons use it alongside sutures. The stitches provide strength, while the glue helps seal the incision, protect against contamination, and may improve the cosmetic appearance of the scar. Sometimes, better healing comes down to the smallest details. ✨🩹 This version avoids overstating what surgical glue can do while remaining engaging.

A visible dorsalis pedis pulsation such as this can be a subtle but important clinical sign, often associated with peripheral vascular changes or fluid overload in conditions such as right-sided heart failure. In this case, the distended veins and exaggerated pulsation may reflect elevated central venous pressure (CVP), which is frequently seen in congestive heart failure (CHF). Always assess bilateral pulses, look for edema, and correlate blood pressure with cardiac examination findings.
