This is a 70y old lady had a syncope during Tawaf in Hajj. Transferred to the mobile cath lab in the Haram hospital in 10 minutes. Echo done immediately and revealed severe BAV and angio was normal coronary. Transferred same day to King Abdullah Medical City in Makkah. Next day got the TAVI and the day after went to Arafah to perform Hajj which was uneventful. We are setting a new benchmark in mass gathering medicine. 80y♀️ presented with CP, hypotensive, and bizarre diffuse ST elevation . She had previous LAD stenting 4y ago. Angio revealed distal LM stenosis, osteal LAD stenosis (ruptured plaq) and osteal LCX. Nanocrush stenting of the LCX with crossover LAD to LM stenting. IVUS guided and only OPN super high pressure balloon was used. 14y boy with blunt trauma to chest after MVA, results in ribs fracture, lacerations to the lung, esophagus, trachea. #YesCCT revealed tear to aorta with pseudoaneurtsm formation, leaks to mediastinum, & fistula to LPA. BP 70/30, Hb 7. There is no landing zone for TEVAR & small femoral artery. Tear starts at the origin of bovine trunck. Multiple BeGraft aortic from Bentley used (covered balloon expandable) is used successfully. The LSCA was covered after making sure that right VA is dominant with retrograde filling of LSCA from left VA.

Interested case to share💡 24y♀️ Primigravida, 23 week of pregnancy P/w with classic features of Preeclampsia and very high BP190/110 and pulmonary edema. The BP was very difficult to control with (labetalol, nifedipine or methyldopa). In fact the fetus start to have Bradycardia from medication. Clinical examination and echo revealed undiagnosed BAV and aortic coarctation (60mmHg gradient) Coarctaion stenting was done with minimal radiation & contrast. Feto-Maternal status improved immediately. Preeclampsia is common in Patient with Coarc and carries significant risk of feto/maternal complications, higher than non-Coarc. It was so difficult to advance the 14Fr Mullin sheath through the pre-dilated 22 Contegra conduit (with 20mm Z-med 🎈) to do pre-stenting with BeGraft Aortic Bentley covered stent to implant #Melody valve 22mm. We used the BAT (Balloon assisted tracking) with #Armada🎈8×80mm to advance the sheath and to fix the sheath while unfolding the loop. It worked like a charm. I love ❤️it. 85y♂️ NSTEMI, 5y ago had TAVI in embolized TAVI during same procedure. Today, PCI to RCA & angiogram to LM done (LAD non-viable). 5Fr RJC advanced over wire between the valves and aortic wall. Spacious STJ was helpful.
