Can you imagine this RCA was completely occluded? We... recently treated a young patient with a chronic total occlusion (CTO) of the RCA. Given the poor antegrade options and well-developed collaterals, we opted for a retrograde approach. After successful crossing, lesion preparation was followed by a "leave-nothing-behind" strategy using four drug-coated balloons (DCBs)—no stents implanted. The image you can see below is a follow up performed 3 months after the CTO PCI. Why this approach? Preserves native vessel function and vasomotion Avoids permanent implants and associated long-term risks Reduces need for prolonged DAPT Makes future interventions easier, especially valuable in younger patients.
