Can you imagine this RCA was completely occluded? We... recently treated a young patient with a chronic total occlusion (CTO) of the RCA. Given the poor antegrade options and well-developed collaterals, we opted for a retrograde approach. After successful crossing, lesion preparation was followed by a "leave-nothing-behind" strategy using four drug-coated balloons (DCBs)—no stents implanted. The image you can see below is a follow up performed 3 months after the CTO PCI. Why this approach? Preserves native vessel function and vasomotion Avoids permanent implants and associated long-term risks Reduces need for prolonged DAPT Makes future interventions easier, especially valuable in younger patients. 20y/M , Massive Unprovoked PE , systemic thrombolysis done... after 48 hours of symptom onset . Unfortunately PA pressures still remain high at 55mmHg on echo and large thrombus burden in RIGHT Common Iliac upto popliteals.. Catheter embolectomy performed for PE and clots are removed Interestingly enough there is Peripheral PS in Right segmental PA .. Not affordable for Penumbra for DVT .. What next for him ?
