Actually they cast shadows, just those are filled by other lights. It is like a football stadium at night, lights hit from every angle so shadows get filled in instantly.

its bullshit we constantly have shadows in lumbar disc hernias with small incisions. Maybe abdominal surgery with large incision. This is why we need endoscope sometimes
