There is a TINY percentage of people with true gender dysphoria, the actual diagnosed mental disorder. SOME of them transition as a form of mental health therapy. Essentially, they know they aren't the opposite sex but living as if they are relieves some symptoms like anxiety. They ALL act this way. Their mental disorder isn't delusion, it's the dysmorphia. They can integrate the fact they ARE male, but only LOOK female. That's OK for them because it helps the anxiety, or whatever their symptoms are.
