Looks like a tail rotor problem.

Bank into the direction of the turn to increase radius and eventually airspeed and tail rotor effectiveness while allowing a gradual reduction in torque thus increasing tail rotor effectiveness. Think Reverse corkscrew. His rpm of the rotor was too low, and got below the threshold for control at that altitude. High altitude operations require a lot of training and experience to avoid this.

The guy in the foreground should have been a bit more expeditious in his departure. I've seen a main rotor pop off and fly about 200 yards after the helo hit the water, although much harder and with no tail rotor.
