What if We're Using GLP-1 Medications All Wrong? Drugs like Ozempic, Wegovy, and Mounjaro are transforming the landscape of medical weight loss, but could their side effects be a sign that we’re not harnessing their full therapeutic potential? In this eye-opening conversation, Dr. Ben Bikman (Benjamin Bikman), metabolic health researcher and professor at BYU, joins Dr. Bret Scher (Bret Scher, MD) to explore a powerful new framework: using GLP-1 medications at low doses and for short durations to help curb carbohydrate cravings, break addictive eating cycles, and support long-term metabolic health. Rather than prescribing high doses indefinitely, Dr. Bikman proposes a more targeted approach: - Microdosing GLP-1s to enhance satiety and reduce cravings for processed carbs - Using the medication as a temporary metabolic tool to support transitions to lower-carb diets - Reducing long-term risks such as muscle loss, mood changes, and diminishing effectiveness - Emphasizing the importance of habit change, insulin regulation, and muscle preservation This conversation reimagines GLP-1s not as a lifelong solution, but as a catalyst for sustainable, low-insulin lifestyles, aligned with ketogenic and metabolic therapies. 📌 Could a 90-day microdosing protocol replace years of medication? Learn how this metabolic-first strategy could empower patients to reclaim their health, without becoming dependent on medication for life. #GLP1 #Ozempic #Wegovy #Mounjaro #BenBikman #MetabolicHealth #Microdosing #InsulinResistance #LowCarbDiet #KetogenicTherapy #MedicalWeightLoss #BretScher #MetabolicMind Expert Featured: Dr. Benjamin Bikman Benjamin Bikman Papers/Articles Mentioned: Follow our channel for more information and education from Bret Scher, MD, FACC, including interviews with leading experts in Metabolic Psychiatry. Think your blood glucose tells the whole story? Think again. Dr. Ben Bikman (Benjamin Bikman) joins me to discuss insulin resistance, including why it often remains hidden until late stages and simple ways to detect it early. We explore insulin’s lesser-known roles in fat storage, appetite regulation, inflammation, and chronic disease. You’ll also learn how macronutrients, meal timing, and frequency uniquely influence your insulin sensitivity, and the critical differences between visceral and subcutaneous fat. Finally, we dive into how stress, sleep deprivation, nicotine, and environmental toxins shape your metabolism and unpack the realities of popular GLP-1 receptor agonist medications (like Ozempic), including their benefits, risks, and the intriguing potential of microdosing. We also provide practical, actionable strategies, dietary adjustments, exercise protocols, and targeted supplements that you can implement today to significantly boost insulin sensitivity and support long-term metabolic health. If you want to understand insulin resistance and take immediate steps to improve your metabolic health - this conversation is a must-watch. There’s been a lot of buzz—and confusion—about the new Keto-CTA study, examining plaque progression in Lean Mass Hyper-Responders (LMHRs). Much of the social media debate has centered on whether high LDL on keto is safe or dangerous, driven largely by how to interpret the supplemental table comparing this study to others on LDL and plaque progression. In this episode of the Metabolic Mind Podcast, we sit down with Dr. Matthew Budoff, a world-renowned cardiologist, cardiac CT researcher, and the study's lead investigator, to discuss the the supplemental table, what the plaque markers mean, and how this fits into the discussion of high- vs -low-risk plaque progression. In this episode, we cover: ✅ What PAV (Percent Atheroma Volume) is, what it actually measures, and why it matters ✅ Why a 50% increase in plaque may sound scary, but can be deceiving ✅ The difference between “treatment-naive” and “treated” participants ✅ What the Miami Heart Study comparison reveals about keto, LDL, and plaque ✅ Why LDL alone may not tell the whole story about heart disease risk ✅ How some high-risk individuals may still benefit from statins and other therapies This study doesn’t answer whether keto causes heart disease or not. Instead, it shows that high LDL on a ketogenic diet is not a reliable predictor of plaque progression across all individuals. What is predictive? The presence of existing plaque. 💡 Key takeaway: Relying on surrogate markers of heart disease, like LDL and ApoB, is not the best way to assess heart disease risk in all populations. If you're concerned about how elevated LDL may be affecting your heart health, the best next step is to speak with your doctor about cardiac imaging to directly assess plaque and gain a clearer picture of your individual risk. The Politics of Food Guidelines and the Real Food Revolution | Marion Nestle, PhD In this episode, legendary food policy expert Dr. Marion Nestle (Marion Nestle) joins the show to discuss her new book "What to Eat Now" and share her unfiltered take on the 2025 Dietary Guidelines for Americans. At 89 years old, Dr. Nestle brings over five decades of experience to the conversation, including her role as editor of the 1988 Surgeon General's Report on Nutrition and Health and as a member of the 1995 Dietary Guidelines Advisory Committee. We explore how supermarkets are designed to maximize spending rather than health, why the food industry's profit motive conflicts with consumer wellbeing, and why the simplest nutrition advice—eat real food, not too much, mostly plants—remains so difficult to follow. We also dig into the controversy surrounding the new dietary guidelines, examining both what Dr. Nestle praises (the emphasis on real food and limiting ultra-processed products) and what concerns her (the meat-forward messaging and questions about equity). As a member of the Scientific Review Group that developed the scientific foundations for these guidelines, I offer an insider perspective on what the evidence actually says versus how it's been communicated. The conversation turns to implementation challenges—from school meals to SNAP policy—and asks the critical question: how do we translate good dietary guidance into meaningful change when funding for community programs is being cut? If you're interested in understanding the forces shaping American food policy and what it means for public health, this episode offers a candid, nuanced discussion from two experts coming from different perspectives on the debate. Testosterone is far more than just a muscle-building hormone. It’s a cornerstone of health influencing mood, cognition, bone density, libido, and even longevity. In this episode, Derek (More Plates More Dates), host of the popular YouTube channel ‘More Plates More Dates' and co-founder of Marek Health, joins me to unpack testosterone’s true role in health, from bone density and insulin sensitivity to brain function and longevity. We cover how lifestyle factors, diet, and micronutrients like vitamin D, zinc, and magnesium shape testosterone levels, and which testosterone-boosting supplements actually work versus those that are just hype. We dive deep into testosterone replacement therapy—when it’s warranted, what benefits to expect, the risks (like polycythemia and fertility suppression), and how delivery methods such as injections, creams, and orals formulations differ. We also explore testosterone therapy for women, cardiovascular disease and prostate cancer risk, and how DHT drives hair loss, plus the safety and efficacy of treatments like finasteride, minoxidil, microneedling, and ketoconazole shampoo. If you want to understand testosterone beyond the headlines, including its functions, its tradeoffs, and the practical ways to optimize it, this conversation is a must-listen. Links in the next post.
