In WPATH's Own Videos, Doctors Describe Billing Gender-Transition Care Under Alternative Diagnoses A psychiatrist called switching to "unspecified endocrine disorder" a way to "work around" the system. A Utah OB-GYN said she billed that way for 14 years. The Trump administration calls such coding fraud. When New York City’s Mt. Sinai Health System reached a settlement with the Justice Department last week to keep its pediatric gender clinic closed, much of the public debate over why the hospital had capitulated focused on the Trump administration’s efforts to obtain patient records via subpoena. This overlooked the mounting efforts by Republican lawmakers more broadly and the Trump administration in particular to leverage accusations of fraudulent medical billing to target doctors in this field and drum them out of providing gender-transition interventions to minors. A spokesperson for Mt. Sinai said last week: “The settlement is complex, but at its core reflects Mount Sinai’s commitment to safeguarding our patients’ confidential information and protecting our clinicians.” Those last few words, protecting our clinicians, at least theoretically could refer to protecting clinicians from prosecution or civil lawsuits for the use of non-gender-identity-related diagnosis and procedure codes when seeking reimbursement for puberty blockers, cross-sex hormones or gender-transition surgeries to treat gender dysphoria in minors. Last year, I obtained several hundred videos from conferences held by the World Professional Association for Transgender Health and its US affiliate, USPATH, between 2021 and 2023. These videos were subject to a subpoena by the Alabama attorney general, Steve Marshall, as part of his defense in a lawsuit against the state’s ban on gender-transition interventions for minors. I covered how these videos portrayed the besieged field of pediatric gender medicine in a report for Compact magazine earlier this year and have been serially publishing key videos on my Substack since then. I have newly scoured these videos for conference speakers addressing the use of alternative procedure or diagnosis coding in transgender care. I found a number of examples, video clips of which I have published and analyzed below. None of them included speakers specifically referring to minors in the moments when they made such references to coding choices, mind you. But in at least some cases, the wider context of their presentations or their response to audience questions did not rule out that minors could be included in what they were suggesting or recalling about coding practices. In American medicine, physicians use diagnosis codes from the ICD system, which specify the condition being treated, and procedure codes known as CPT codes, which indicate the actions the physician took to treat the condition. These codes are integral to obtaining reimbursement for services. Insurers require diagnosis codes to serve as an appropriate justification for the corresponding procedure. The U.S. Department of Health and Human Services on Aug. 13 published a scathing report bearing the incendiary title “Wolves In White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of ‘Gender Medicine.’” Much of the report is devoted to examining how leaders in pediatric gender medicine advocate the use of alternate codes when seeking insurance reimbursement for gender-transition interventions. These practices, the report asserts, have become widespread in this field. The report serves as an ominous warning to health care providers still practicing in what remains of a field that the Trump administration has seen considerable success in destroying outright. “When healthcare providers use fraudulent medical coding to obtain payment from the federal government,” the HHS report asserts, “they may be liable under the federal False Claims Act, which carries fines and imposes damages of ‘3 times the amount of damages which the government sustains.’” To follow federal regulations, the report asserts, physicians treating patients for transgender-related purposes are meant to use a number of ICD diagnosis codes related to gender identity, sex reassignment or “dual role transvestism.” But it has become common for medical providers to use alternate diagnosis codes, according to the report—in particular those related to endocrine disorders. This includes, the report states: “Endocrine disorder, unspecified,” coded as E34.9. Using this code in place of a gender-related diagnosis (e.g., gender identity disorder, F64.0) violates CMS guidelines for the simple reason that gender dysphoria is not an endocrine disorder, since it exhibits no hormonal abnormalities. In fact, the only endocrine disorders these patients have are the ones created by their doctors through the use of sex-rejecting interventions. Yet it appears that healthcare providers are misusing this code. In a 2016 studytitled “Identifying the Transgender Population in the Medicare Program,” CMS’s Office of Minority Health acknowledged that proxy diagnosis codes are being substituted for gender dysphoria. In the methods section, the authors explain that they included the ICD-9 diagnosis code for Unspecified Endocrine Disorder in their analysis because it is “frequently used by the transgender community to combat the perceived stigma of a GID diagnosis.” The report goes on to note that a 2023 study also finds such use of alternative billing codes. The report further notes an analysis of medical-claims data by Leor Sapir, a senior fellow at the Manhattan Institute who studies pediatric gender medicine, that found an almost 30 percent increase in endocrine disorder, unspecified diagnoses in minors between 2020 and 2022. Dr. Sapir asserted in 2025 that this was likely the product of a “greater willingness” by medical providers to use endocrine disorder codes over gender identity disorder codes in pediatric care of patients with gender dysphoria. The authors of the HHS report analyzed health insurance databases covering 2015 to 2025 and found that insurers were billed nearly $50 million for puberty blockers for patients age nine to 17 under endocrine disorder codes, excluding precocious puberty. There was nearly $11 million in billing for 13- to 17-year-olds using a precocious puberty diagnosis code. The report asserts that “this data strongly suggests a pattern of potentially fraudulent billing that relies on miscoding for endocrine disorders to justify the prescription of puberty blockers.” WPATH, the report goes on to state, has promoted “the misleading use of several ICD-10 codes” in its training presentations. Teddy Goetz Dr. Teddy Goetz is a psychiatrist who currently works at the Mid-Atlantic Permanente Medical Group. Dr. Goetz, who identifies as nonbinary, was a coeditor of a controversial text put out by the American Psychiatric Association’s publishing arm in 2023 outlining how to apply the gender-affirming method to the care of what these days are known as gender-expansive patients. Dr. Goetz spoke at USPATH’s 2021 conference. The moderator, taking an audience question, asked about how to move away from a distress-based model of care for transgender patients, in which it is required that they harbor distress related to their gender identity to qualify for a diagnosis that will justify gender-transition treatment. “We can think about it in the terms of billing codes,” Dr. Goetz replied. When a patient first presents, Dr. Goetz said, a doctor could use a gender dysphoria-related billing code to process a prescription for gender-transition treatment such as testosterone. But if this treatment ultimately attenuates the patient’s dysphoria, Dr. Goetz said, “Then you could switch your billing code to ‘unspecified endocrine disorder,’ for example, saying that their body produces less testosterone than it should for them to feel their best. So there are different ways that you can work around that. And thinking about moving towards euphoria and away from dysphoria.” Addressing a common conundrum that expands well beyond transgender medicine, Dr. Goetz continued: “But our current health care system in the United States is really set up against a preventive health care model and towards more of a responding-to-active-problems model.” It remained unclear whether Dr. Goetz was referring to treating pediatric patients when describing these billing practices. What is evident, however, is that Dr. Goetz, who at the time was a medical resident at the University of Pennsylvania, was then working at least part of the time at a children’s hospital. At one point during the conference session, Dr. Goetz made a specific reference to “the children’s hospital where I do work in addition to the adult hospital.” A spokesperson for Kaiser Permanente provided me a statement indicating that “We understand” that Dr. Goetz “was speaking hypothetically” at the conference. “Dr. Goetz is an adult psychiatrist and does not provide care to, nor prescribe puberty blockers or hormone therapy to Kaiser Permanente pediatric patients,” the statement continued.

No study has been able to prove such a causal link. Studies have only found associations between these treatments and higher mental health metrics. ▶️Going along with the CNN anchor's misleading characterization of the notion that these treatments "have been used for decades." Until 2008, only about 120 minors in the world had had their puberty suppressed for gender dysphoria. It wasn't until about 2013 that cases of pediatric gender dysphoria shot up and these treatments started being used widely. So there really has only been about a decade of substantial use of the drugs. ▶️Falsely claiming we know that regret rates are low among those who received gender-transition interventions as minors. There has been woefully insufficient research on this subject, and follow-up has been too short and too sparse to make such definitive statements. In the fall of 2022, Jon Stewart had an argument with the Attorney General of Arkansas about that state’s ban on using drugs and surgeries in an effort to change the gender of children and adolescents. “You’re suggesting that protecting children means overriding the recommendations of the American Medical Association, the American Association of Pediatrics, and the Endocrine Society,” said Stewart. And he was right: those major medical organizations had indeed endorsed what’s called gender-affirming care. But the attorney general of Arkansas was also right when she said there was never sufficient evidence for them to do so. It’s true that gender dysphoria and gender distress are indeed serious psychological and psychiatric problems. But there has long been strong evidence that other things, including autism, anxiety, and just being gay, are being misdiagnosed by children, parents, therapists, and doctors as gender dysphoria. And now there are the newly released internal discussions from the World Professional Association of Transgender Health, known as WPATH. They show that trans activists aggressively pressure doctors and therapists for the medicalization of gender dysphoria with drugs and surgeries before first attempting to address autism, anxiety, and other psychiatric disorders. The question now turns to why the American Medical Association, the American Academy of Pediatrics, and the Endocrine Society advocated drugs and surgery for gender distress despite the lack of evidence. There’s a short and a longer answer to that question....

'I want my vote back': Riverbend residents furious after Matt Jeneroux defects to Liberals It was –20°C and snowing in Edmonton Riverbend, and with the help of more than 22,000 petition signers, Rebel News deployed a billboard truck through the riding to demand accountability from Matt Jeneroux after he crossed the floor to the Liberal Party. Riverbend has voted Conservative in every federal election since the riding was created in 2015. And now, without a by-election, it has a Liberal MP. Back in November, rumours surfaced that the Liberals were pressuring Jeneroux to cross the floor. When the backlash began, he publicly denied it. Soon after, he announced he was resigning, citing family reasons in what looked like a long goodbye. Less than four months later, he resurfaced at an airport press conference alongside Prime Minister Mark Carney — announcing he was joining the Liberals. He cited Carney’s speech in Davos as the moment that changed his mind. That was the explanation. No town hall. No constituency-wide consultation. No vote. Just the ol' Liberal Switcheroo. “I voted for him. I want my vote back.” Residents didn’t hold back. Two of the most powerful medical institutions in the United States just blew up one of the left's most protected sacred cows. The American Society of Plastic Surgeons and the American Medical Association have now both gone on the record saying the same thing: there is insufficient evidence to justify gender transition surgeries for minors, and these surgeries should generally be deferred to adulthood. That's not conservative media, that's not Republicans, that's not Donald Trump making a social media post. That is the medical establishment finally backing away. The plastic surgeons, eleven thousand of them, now warn that chest, genital, and facial surgeries for minors are irreversible and unsupported by solid long-term evidence. And the AMA, for the first time, is saying out loud what trans cult activists spent years trying to suppress, that we don't actually know if any of this helps kids, and we shouldn't be mutilating them while we figure it out. This is a total collapse of a narrative. Now, the AMA is enormous. The AMA, the American Medical Association, was among the leading groups opposing laws trying to restrict gender-affirming surgeries for minors just a few years ago, and the media would regularly cite the AMA to create an impression of expert consensus on this issue. The current shift isn't based on any significant change in the data available. The data really has always been nonexistent. This all speaks volumes about how previous recommendations that ruined countless lives were based on politics instead of sound science.”

Two of the most powerful medical institutions in the United States just blew up one of the left's most protected sacred cows. The American Society of Plastic Surgeons and the American Medical Association have now both gone on the record saying the same thing: there is insufficient evidence to justify gender transition surgeries for minors, and these surgeries should generally be deferred to adulthood. That's not conservative media, that's not Republicans, that's not Donald Trump making a social media post. That is the medical establishment finally backing away. The plastic surgeons, eleven thousand of them, now warn that chest, genital, and facial surgeries for minors are irreversible and unsupported by solid long-term evidence. And the AMA, for the first time, is saying out loud what trans cult activists spent years trying to suppress, that we don't actually know if any of this helps kids, and we shouldn't be mutilating them while we figure it out. This is a total collapse of a narrative. Now, the AMA is enormous. The AMA, the American Medical Association, was among the leading groups opposing laws trying to restrict gender-affirming surgeries for minors just a few years ago, and the media would regularly cite the AMA to create an impression of expert consensus on this issue. The current shift isn't based on any significant change in the data available. The data really has always been nonexistent. This all speaks volumes about how previous recommendations that ruined countless lives were based on politics instead of sound science.”

The true intention of the Minister for Justice and the Department of Justice is to substitute the new definition of gender into every piece of legislation on the statute books that involves gender. This was made clear during the Seanad Debate on Wednesday last week when a private Fine Gael email from Helen McEntee to Deputy Richard Bruton was read out by Senator Ronan Mullen. That email was in relation to the new definition of gender in the Hate Crime Bill. The email stated that a review of the Equal Status Act and the Employment Equality Act is taking place and that when that review is finished, the definitions of protected characteristics in the Hate Crime Bill will be reconsidered to ensure consistency in definitions across the Statute Book. In public and to the Houses of the Oireachtas, the Minister is saying that the new definition of gender is for the Hate Crime Bill only. In private and to her Fine Gael colleagues, she is saying that all definitions of gender across the Statute Book will be rewritten. Such deliberate deceit from one in such a position of responsibility and authority is reprehensible. It calls for the Minister's resignation. We thank God for the recent closure of the Tavistock gender clinic, to which hundreds of vulnerable Irish children had been referred. This Hate Crime Bill with its new definition of gender would give legitimacy to this poisonous ideology which preys on the most vulnerable and allows for no dissent. This is the eleventh hour. Premier Ford declares ALL media unwelcome at PC Convention in Toronto Sources tell us that the Premier is about to get an earful from angry delegates, and apparently, Ford doesn’t want this dirty laundry aired in public. Much like other years, the Toronto Congress Centre is the venue for the annual meeting of the Ontario Progressive Conservative Party. But there’s a new twist to this year’s convention: all members of the media are persona non grata. Ontario PC spokesman Peter Turkington issued a terse statement the other day: “The Ontario PC Party Convention is a closed event and will not be open to media attendance.” But the question arises: Why? Well, no reason was given. That’s odd. The convention used to be open to the media. Actually, correction: In recent years, the Ontario PC Party inexplicably banned independent media from its conventions, including Rebel News. It's all very odd, given that Doug Ford used to attend Rebel News Live events as a guest speaker. Of course, that was before he became Premier. Since then, Ford cut ties with us. And again, no reason given. In any event, we are left to speculate what the unspoken reason is for the all-encompassing media ban this year. Sources tell us that the Premier is about to get an earful from angry delegates, and apparently, Ford doesn’t want this dirty laundry aired in public. We were also told many party members remain upset that Ford did his best to undermine Conservative leader Pierre Poilievre in the last federal election, and that continues to do so now. Just consider the date for the Ontario PC Party Convention: the same weekend the federal Conservatives are assembling in Calgary for their convention. That forced many Ontario PC members to choose which convention they would attend. And really, is this purposeful conflict not the pinnacle of pettiness for Doug Ford? Nevertheless, Rebel News dropped by the Congress Centre for answers. And get this: as we were heading in, Ford was heading out. We asked him a few questions, although he didn’t give any answers before he scurried away, ensconced within a Chevrolet Suburban. Par for the course, Toronto Police then informed us we could not even be in the parking lot, so we had to leave. Truthfully, they probably did us a favour given the frigid conditions… It seems that Doug Ford, who once ran on the slogan “Respect for Taxpayers”, doesn’t have a whole lot of respect for anyone these days. Andy Beshear wants to be your next president, and if he ever gets the chance, he'll stomp all over Christian values and tell you it makes him a better Christian to do it. He also vetoed multiple bills regarding gender-affirming care and LGBTQ+ rights during his administration including: Senate Bill 83 (2002) - A bill that banned transgender girls and women from participating in school sports matching their gender identity. This veto was also overridden. Senate Bill 150 (2023)- A bill banning gender-affirming medical care for minors, including puberty blockers and hormone therapy. The bill allowed teachers to refuse to use a student's preferred pronouns, restricted bathroom use based on birth sex, and required parental notification for school lessons on sexuality. House Bill 495 (2025) - A bill to prohibit Kentucky Medicaid funds from paying for gender-affirming care (hormones or surgery) for transgender adults. Everyone needs to know just how bad Andy Beshear is. He will be one of the Left's favorites in 2028, and we've got to get the word out early and often.
