The flies landing on his foot are depositing eggs and maggots will grow. The flesh has necrotized. The man is unfortunately very ill and most likely didnt maje it much longer.

This reminds me of a video I saw a few weeks ago of this guy in Seattle with a fucked up foot that was gushing puss. He was hopped up on something, homeless, and refused medical attention. It was quite disgusting. He's probably dead now.

His right leg has been amputated already. The left side has edema, it's as gross as you're imagining. Mostly likely uncontrolled diabetes, heart failure, kidney failure. He'll end up in the ER and they'll amputate the Left side too.
