Gotta say…doesn’t like an easy CTO from the start. 😜. Few comments. When puncturing into aorta I would avoid knuckling into aorta, unless you are certain that wire is luminal at the ostium. This risks aortic wall hematoma formation. If there is any doubt you are subintimal at ostium or prox RCA then puncture with a stiff wire. Also when snaring it’s best to snare dedicated externalisation wires. Looks like you were snaring the knuckled fielder XT wire.

Agree, if new entry in aorta needed, best is Electrocautery and we were prepared for that. Knuckled XT until ostial then used black. Black is in aorta. Knuckling in Neo Ostia will be a bad move as you described!
