Echo From Scratch | Post #16 Parasternal Long-Axis View: Your First Major Window The parasternal long-axis view, or PLAX, is one of the foundational views in transthoracic echocardiography. The patient is generally positioned in the left lateral decubitus position, with the transducer placed near the left sternal border, usually... around the 3rd or 4th intercostal space. The goal is to obtain a plane that displays the LV long axis and the aortic and mitral valves. Transcatheter mitral valve replacement (TMVR) devices that are currently undergoing clinical testing have a high anatomical screen failure rate due to challenges with annulus size or left ventricular outflow tract obstruction. The Saturn valve (InnovHeart) is a novel TMVR bioprosthesis that has a central valve mechanically connected to an annular ring. It features a low-profile design with a small footprint in the left ventricle. The annular structure and the anterior connecting arm immobilize the anterior mitral leaflet, which prevents systolic anterior motion. This is fascinating video concept comes from a study performed to assess the procedural performance and long-term healing profile of this TMVR system in healthy preclinical models.

Apical 4-chamber view: a single window that maps cardiac anatomy and relationships. How to obtain it: 🔵 Place the transducer at the point of maximal impulse (LV apex) 🔵 Keep the probe index marker directed toward the patient’s left side (≈3 o’clock) 🔵 Aim the beam toward the base of the heart (toward the right shoulder) 🔵 Fine-tune tilt/rotation until all four chambers are symmetric and the septum is vertical From the LV apex, you see all four chambers in one plane (LV, RV, LA, RA) with correct orientation (apex near the probe, atria deeper). Key structures: 🔵 LV: papillary muscles, chordae, anterior & posterior mitral leaflets 🔵 RV: moderator band, anterior & septal tricuspid leaflets 🔵 Interatrial septum and ventriculoatrial septum (LV–RA separation) 🔵 Right superior pulmonary vein entering the LA near the septum 🔵 Descending aorta seen posterior/lateral to the LA High-yield pearl: The tricuspid valve annulus is normally positioned more apically than the mitral, an important landmark in structural assessment.
