Transcatheter mitral valve replacement (TMVR) devices that are currently undergoing clinical testing have a high anatomical screen failure rate due to challenges with annulus size or left ventricular outflow tract obstruction. The Saturn valve (InnovHeart) is a novel TMVR bioprosthesis that has a central valve mechanically connected to an annular ring. It features a low-profile design with a small footprint in the left ventricle. The annular structure and the anterior connecting arm immobilize the anterior mitral leaflet, which prevents systolic anterior motion. This is fascinating video concept comes from a study performed to assess the procedural performance and long-term healing profile of this TMVR system in healthy preclinical models.

She is previously healthy, this has been the same size for the last 2 years, intensity of pain has increased recently. In a moment of panic, the sheath was advanced—an action that should never be taken in such circumstances—before transferring the patient to our facility. Upon arrival, we carefully retracted the sheath under guidewire support, prepared for thoracotomy if needed. Subsequent angiography and TEE assessments revealed no evidence of a shunt or bleeding. Fortunately, this severe complication was resolved without the need for surgical intervention. Key Lessons: 👉 1. Always puncture posteriorly; the left atrium is located in the posterior aspect. 👉 2. Remain calm; never advance the sheath into the aorta. 👉 3. Do not retract the guidewire until ruling out any shunt or bleeding🚨🚨 👉 4. An active weather pattern is setting up for Tuesday as a cold front approaches and a warm, humid air mass surges into the area.
