On the submarine, when something went wrong, they ran "critiques" - the five whys, all the way to root cause. "Silicon Valley has traditionally been bad at that because it's just moving so fast... There's this idea that we don't have time to fully unpack that, or we can take a surface level answer for what happened because we'll just build it again, let's move..."
