The issue is that if you put the wide end in first, it feels like it is trapped behind your teeth. Of course, if you can open your mouth wide enough to put it in there, you can get it out again. The problem could be people panicking when it becomes difficult to remove (children etc), and end up pulling it out too quick and breaking it with your teeth.
