DC-10 drops 85,000lbs of Phos-Chek retardant over the Wahluke Slope Fire/Saddle Mountains back in September 2018. When the aircraft suddenly releases that much weight, the biggest immediate effect is on trim. As the airplane becomes lighter, it needs less lift to maintain level flight, creating a nose-up pitching tendency. You can actually see the aircraft begin to pitch up the moment the drop starts. The defect cannot be repaired where the aircraft is, so it must be flown to a maintenance facility. It does not fly on a normal Certificate of Airworthiness. It flies under a Special Flight Permit: a one-time authorization that defines where the aircraft can fly, the crew required, and the operating limitations for that specific flight. I have been part of the process of preparing one of those permits. The defect is assessed. The risks are evaluated. The operating conditions are defined. The crew is fully briefed on the defect and its limitations. The route is planned around those limitations, and suitable alternate airports are selected. Then the aircraft departs, and you watch it leave, knowing exactly what condition it is in. That feeling stays with you. The permit exists because, in some cases, the safest option is to fly the aircraft under controlled conditions to a facility with the right people, tools, parts, and approvals, rather than attempt a repair where those resources are unavailable.

49 years ago today: America´s worst air disaster May 25 1979: American Airlines Flight 191, a DC-10, crashes on takeoff from O'Hare (Chicago, US). All 271 aboard plus 2 on the ground die, the deadliest airliner crash in US, a figure only surpassed by the 9/11 terrorist attacks. On take-off, the engine separated from the wing due to improper maintenance, damaging hydraulics, and causing loss of control, as detailed below. Specifically, it was determined that during takeoff rotation, the No. 1 (left) engine separated from the left wing, flipped over the wing, and landed on the runway. The Federal Aviation Administration is investigating after a flap on a Boeing 737’s wing partially broke off before landing in Texas on Tuesday. Delta Air Lines Flight 1893 was flying from Orlando International to Austin-Bergstrom International Airport when passengers noticed something had partially broken away from the back of the wing. The airline said in a statement, "After landing it was observed that a portion of the left wing’s flap was not in place. The aircraft has been taken out of service for maintenance. NEW: TMZ alleges there was a fifth plane on 9/11 that was covered up by the FBI. In a new documentary, TMZ interviewed 3 flight attendants and the pilot who were on United Flight 23. After being evacuated from the plane, the pilot says they later found that the hatch on the bottom of the plane had been opened. "The airplane was locked and sealed when I left the airplane, so something else was going on that I couldn't explain." The flight crew said there were four "Arabs" on the plane who demanded that the flight take off when flight attendants tried giving them food. One of the men had requested to see the cockpit before takeoff. The crew says they were interviewed by the FBI, but never heard back, and never found out if the men were questioned or not. "We have requested information about Flight 23 ... we did not get a reply," TMZ said.

All 301 people on board perished, and the accident still ranks as the sixth-worst disaster in civil aviation. Why "bizarre"? Wife filed a maintenance case in Kanpur. The husband’s family claimed that he is paralysed, bed-bound, and unable to work for last 5 years, and attached medical documents as proof. The wife and the family court did not agree. The husband’s family then paraded him on a stretcher to make them believe. I doubt the husband’s family will escape liability even after this. The judge can still pass the maintenance liability on the husband’s father.

Staines air disaster was 54 years ago June 18 1972 - BEA Flight 548, a Trident, crashes in Staines (England). All 118 aboard die, the worst air disaster in the UK. After takeoff, lift devices were prematurely retracted, inducing a superstall and loss of control. Factors: crew actions, F/O inexperience, Captain´s heart issues. Staines air disaster was 54 years ago June 18 1972 - BEA Flight 548, a Trident, crashes in Staines (England). All 118 aboard die, the worst air disaster in the UK. After takeoff, lift devices were prematurely retracted, inducing a superstall and loss of control. Factors: crew actions, F/O inexperience, Captain´s heart issues. The accident occurred as the aircraft entered a deep stall during the third minute of flight, and descended steeply before crashing into the ground, narrowly missing a busy main road. The public inquiry primarily attributed the accident to the captain’s failure to maintain adequate air speed and to correctly configure the high-lift devices. We often speak of poor cabin evacuations, such as when pax forcibly BRING their luggage and disobey instructions. This is the opposite: an example of a good one. August 20 2007: China Airlines Flight 120, a B-737, is destroyed by fire in the apron at Okinawa (Japan). All 165 aboard evacuated safely. The inquiry found that improper maintenance on the slat mechanism [a loose bolt] led to puncturing of a fuel tank, causing a leak that started the conflagration.

Stress levels are quite high in the sky these days : An Air Canada flight from Casablanca (CMN) to Montreal (YUL) was canceled due to the inappropriate behavior of a flight attendant on July 26, 2024. The Airbus A330-343 aircraft (C-GHLM) did the taxi aiming a departure while operating flight AC73, but had to return back to gate due to the unfortunate development. As per sources, a passenger asked for a blanket because of the excessive air conditioning, leading the stewardess to call the police to disembark the lady.
