This is the cause of the accident, not root cause. Root cause will be to find why did this happen, was it human error or software / electronic device failure, was this issue known to the authorities, what action was taken or was just put under the carpet.

Main point here is to find, how Coromandel changed tracks- train can not change tracks without the point being changed, either humanly or by track error.
