Hey Bitwise! This is awesome stuff. It looks like Google may have changed the way Google Form HTML is formatted and the demos are broken I went ahead and modified the "labelledByPattern" regex on line 31 of FormService and it seems to have fixed it: For those looking to use this module with the fix: Simply copy this entire gist and paste it into the FormService module of this model! Hope this helps!
