Cardiology has become obsessed with the mantra “lower is better.” Lower LDL, lower ApoB, and you're going to be better off. This simple tagline misses the forest for a stupid little cholesterol shrub. It’s well known that intervening to lower LDL and ApoB doesn’t necessarily result in better overall outcomes for the whole person, even in circumstances where cardiovascular risk might be reduced. It's therefore incredibly sad when "certain cardiologist experts" (heavy quotes there) make false claims like: "people with a PCSK9 loss of function mutation have very low LDL-C and live 15-20 years longer than those who don't. And we have meds now that cause that effect." That nonsense I could forgive. What is far more troubling is that there's a general failure to challange problematic (and sometimes outright false) narratives like this provided they align with the status quo, even when the status quo has failed public health miserably. We need to broaden the aperture of the discussion because the "science is not settled." I'm not a doctor and neither is this man but hundreds of hours of personal research lead me to agree with most of his cholesterol panel dissection. Total cholesterol: Fairly useless. LDL is not the boogeyman (although glycated LDL can be a problem) Triglycerides divided by HDL is the best predictor of heart disease risk in a cholesterol panel.

$The pathway to heart disease goes through the mitochondria. It's not LDL cholesterol, nor very fine LDL fraction, nor ApoB that best predicts risk of heart disease. It's blood glucose. Spikes in blood glucose cause glycation -- attachment of a glucose molecule to lipoprotiens -- that weaponizes cholesterol against your arteries. Smaller cholesterol particles are more susceptible to glycation, sure. BUT without the blood glucose spikes resulting from insulin resistance, there's no increased risk of heart disease. Historian Niall Ferguson: Despite risks, overthrowing the regime in Iran would be a positive step for Iranians, the region, and the world. “Is there a risk to toppling Khamenei? Yeah, there is. Is there a risk to leaving the guy in power and whoever succeeds him when he finally croaks? There’s a big risk to that. I would say, on balance, it’s a bigger risk to leave this regime in place.
