It was an insightful session today at the World Economic Forum World Economic Forum as I joined other global leaders, including Anita Zaidi Anita Zaidi of the Bill & Melinda Gates Foundation, Catherine Russell Catherine Russell of UNICEF, and Bernd Montag Bernd Montag of Siemens Healthineers Siemens Healthineers, to discuss the critical importance of investing in women’s health. It also provided an opportunity to share Nigeria’s bold reforms and achievements of HE President Bola Ahmed, GCFR, Bola Ahmed Tinubu under the Health Sector Renewal Investment Initiative (#NHSRII). Our approach integrates innovation and insights from women themselves. For example, focus groups in northeastern Nigeria revealed cultural misconceptions that limit the use of maternal health services, underscoring the need for policies grounded in the lived realities of women. Additionally, we are leveraging Nigeria’s mixed health system by working with the private sector to localize the production of reproductive health commodities and expand economic opportunities for women through job creation in healthcare. We are also prioritizing women’s leadership in healthcare governance through initiatives like the National Health Fellows Program, ensuring women are at the forefront of shaping inclusive policies. These efforts are driven by our commitment to equity and reflect the broader Renewed Hope Agenda’s whole-of-society / whole -of-government approach to achieving better health outcomes for all Nigerians. Section 27 of the South African Constitution is unambiguous. It clearly says healthcare is a right for everyone in the country, regardless of nationality, and that no one may be refused emergency medical treatment. That right extends beyond emergencies, covering access to public hospitals and clinics for foreign nationals who are legally in the country. It is not proper to have mobs in controlled environments like hospitals, because doctors and nurses end up in fear of these mobs. Ordering them to deny people hospital treatment is asking them to violate their professional oaths. Nobody supports illegal immigration, but you cannot stop people who are in South Africa legally from accessing hospitals; your law says they can access them. If you do not want them to use these facilities, change the law instead of taking the law into your own hands. It is unthinkable that, in a civilised society, a random person wearing a political party T-shirt would order doctors to remove people from hospital beds. Embarrassing stuff to watch! It is unlawful to deny anyone emergency medical care, and to forcibly remove them from a facility is assault and possibly attempted murder. We all know that excluding sick people from treatment increases the risk of massive disease outbreaks, which do not respect borders or immigration status. This harms everyone, including South African citizens. Public health depends on treating everyone who needs care, because untreated illness spreads, harming all. I want to emphasise that it is the conduct of a lawless state to deny a legal resident access to treatment at a public health institution. If Gayton McKenzie’s PA does not want foreign nationals who are in South Africa legally, he must push to change the law in Parliament instead of gaslighting people, some of whom do not even know what the law says and might end up in trouble. Only opportunistic politicians would do that; no sane person would use violent mob tactics to address something that is constitutional and has not been challenged in the South African parliament. We all know that denying legal residents healthcare is not just unlawful, it is a direct attack on the Constitution; and when mob rule replaces the rule of law, everyone’s rights are in danger. What is my view on Trump’s executive order pulling America out of the World Health Organisation? President Donald Trump also signed an executive order temporarily suspending all American foreign assistance programs for 90 days, pending reviews to determine whether they align with his policy goals. America provides an average of US$50 billion annually to foreign aid programmes. This means there will NOT be any new financial aid coming from America to anywhere in the world for the next three months, but in this post, I will focus on America’s withdrawal from the WHO, and will talk about about the global impact of the temporary AID ban in days to come when there is enough clarity on the effect of the ban. America is the biggest economy in the world, or possibly the second largest after China, so its contribution to the WHO, though significant, is not irreplaceable. Its contribution amounts to an average of US$120 million annually in the past ten years. That equates to roughly 1.3 tonnes of gold, a resource that Africa has in abundance. Trump’s decision does not prevent other countries from stepping in to fill the gap left by America’s withdrawal. It is not as though the US was solving a problem no other nation could address. From Africa to Europe to Asia and the Americas, others are capable of taking responsibility too and fill in the void left by America. The real danger, however, lies in the possibility that irresponsible and corrupt leaders may fail to step up and address and fund critical issues, such as the fight against HIV and AIDS. If such leadership fails, the world—including America—could face a resurgence of new infections like HIV, undoing years of progress. This would be a massive tragedy for America as well, it won’t be able to insulate itself from resurgent infections short of sealing its borders which it can’t. In today’s highly mobile world, it is impossible to isolate one country from the consequences of a global health crisis. HIV has been controlled through antiretroviral medication, which prevents HIV carriers from infecting others. If access to these medications is disrupted, everyone, including Americans, would be at risk. This could even force the US to return to old practices such as requiring HIV status disclosure before issuing visas to enter and live in America. Unfortunately, Africa is riddled by corrupt and heartless leaders who rob and loot from their people and seek medical assistance outside the continent using taxpayers money. While it is logically possible for African nations to fund their own HIV treatment programmes and infectious diseases control, these leaders’ greed often leaves their populations to suffer needlessly. Another downside of America’s withdrawal is the loss of its technical medical expertise. As the executive order for withdrawal from the WHO outlines, American personnel working with the WHO will be withdrawn within 12 months from today. The US is a global leader in science and technology, and the loss of its knowledge and contributions will be felt worldwide especially by the poor in the Global South. This situation could, however, provide an opportunity for countries like China to step into the leadership vacuum left by America. Nature doesn’t like a vacuum, but Africa, lacking the required technological expertise and plagued by poor governance, is unlikely to fill this gap although it can and should fill it. It is easier for Africa’s middle class to applaud Donald Trump for this move, but it is the poor, who have no voice, who will suffer the most because they lack the means and ability to remove corrupt governments. One child was dying every fifteen minutes from HIV and AIDS in Zimbabwe only 15 years ago until the world intervened. This could become the reality once again unless Africans take action to force their governments to act responsibly for once! So while Trump’s withdrawal from the WHO poses challenges, it also highlights the need for Africa to reassess its priorities and build resilient healthcare systems. Addressing corruption and poor governance is vital and an urgent matter for Africa, but this must be coupled with actionable solutions such as regional health funds, stricter accountability for public health spending, and partnerships with international organisations that prioritise transparency. Countries like Botswana, with its effective HIV programmes, demonstrate that progress is possible even in resource limited circumstances if a country has the right leadership, but for that to happen, you need a different type of mindset and less corruption on the continent. Collaboration between governments, private sector, and civil society can also ensure that health crises like HIV/AIDS are addressed sustainably, and by taking responsibility for their own health systems, African nations can reduce their heavy reliance on external aid and ensure better healthcare outcomes for their populations. America First as a doctrine provides a generational opportunity to lessen America’s global influence and create multilateral platforms where world leaders can genuinely provide solutions for their people! This is an opportunity for groups like BRICS to step up and plug the funding deficit, otherwise, failure to do so will expose them as nothing more than paper tigers that rely on American financial benevolence while bellowing empty slogans! So ultimately, the issue is no longer about America’s withdrawal, it is about the world’s willingness to take full responsibility, with each country prioritising the national interest of saving lives before the looting of public funds. The world has enough to go around, what it only lacks are empathetic leaders who put the health of their people ahead of their personal pockets. Trump will be a four-year global phenomenon, so the world must make a plan instead of mourning about what many in the global health fraternity have labelled a reckless and populist decision that, in the long term, won’t benefit America much! Those who support America’s withdrawal from the WHO should note that it is not just about money, but also about American medical expertise, technology transfer from the US, providing global leadership during pandemics, and having a united world when tackling healthcare disasters like COVID-19, HIV/AIDS, and Ebola. Lastly, if poor African countries run by greedy, corrupt dictators end up with shortages of antiretroviral medicines, countries like South Africa, Namibia, and Botswana, which have well-run programmes, would end up carrying the burden of cross-border health tourism. “In the 15th century, Asia was the most technologically developed region. Europe had advanced only in a few sectors, including gunpowder, cannons, & ocean-going ships. These three technologies helped Europe to conquer the rest of the world. The conquest of Asia & Africa set back the development of science & technology in our countries, creating the technological divide of today. Another quantum leap in technology & innovation is taking place in the 21st century at a geometric rate, further widening the existing technological divide & leading to the marginalisation of the South. Two key factors are essential for developing nations to come out of this crisis. The first is digitalisation & adopting new technologies such as Big Data, Internet of Things, AI, Blockchain, biotechnology, & genome sequencing. The second is ensuring a workforce skilled in the multifaceted fields of science & technology. A well-equipped & knowledgeable workforce is necessary to accelerate the needed transformation. Let me outline the measures my government is taking. A continuing economic restructuring process after the financial crisis of 2022 allows us to harness the potential of science, technology, & innovation for our future trajectory of sustainable development & high growth. We are currently evaluating the performance of our existing science & technology research institutes, some of which are underperforming. To promote utilising new technologies for Sri Lanka's growth, the government will establish a Technology & Innovation Council & a digital transformation agency. Our universities currently do not have the capacity to create the requisite number of technology specialists. Therefore, we will establish four new universities focusing on new technologies. One of these is the outcome of technological cooperation between India & Sri Lanka. The International Climate Change University will be the fifth. The digital divide presents significant challenges to the development of the global south. These include limited access to costly technology, inadequate digital skills & infrastructure, cultural & institutional barriers, & financing constraints. Global trade barriers further compound these issues. To overcome these challenges, the G77 & China must establish an effective mechanism for cooperation. Otherwise, our endeavours will be limited to immediately reinvigorating the consortium of science & technology & innovation for the South, commonly known as COSTIS. This will be a test of our commitment. A revised Programme of Action with other proposed new measures is required. These measures should include collectively creating technological platforms modelled on the European Union's technology & innovation platforms in the following fields: digitalisation, health medication, AI, & renewable energy, including green hydrogen. Secondly, interested G77 countries must commit to earmarking 1% of their GDP for R&D. This has to be achieved over a decade. Thirdly, we must foster collaboration between governments and the private sector. The brain drain from the South to the North & the resulting loss of educated manpower is another threat to the development of science, technology, & innovation in the South. China, India, Japan, & South Korea have developed science, technology, & innovation by nurturing their own manpower. Therefore, we must ask for compensation from the North for the loss of our manpower. Strengthening South-South cooperation can also play a significant role in addressing the critical issues of expanding our human resources. Sri Lanka urges the Group of 77 & China to work together to introduce a scheme similar to the Colombo Plan to increase collaboration, exchange best practices, & push policies that harness the transformational potential of science, technology, & innovation amongst us. Sri Lanka is committed to supporting the new Hawana Declaration. We must ensure that the collective voice of G77 & China is heard in international fora.”
