Every auto first aid kit has a seat belt cutter. Specifically for when belt's tightener goes into the "accident happened, tighten it up" mode. They look like this:

the ONLY way that's possible, is if she laid down while buckled up and twisted (spun around) the other way sitting back up ... she put a twist in it and there's NO WAY she didn't know how she did it
