I believe the Black Eye Club is a ritual initiation. In Egyptian mythology, Horus lost his left eye in a struggle with his uncle Set (Satan). The left eye represents the moon, while the right represents the sun. His eye was restored by Hathor/Thoth and became a symbol of restoration. Thoth, the god of the moon and wisdom, signifies enlightenment. Through this initiation, members symbolize wisdom and restoration. In the occult, symbols are frequently used due to the belief in the power they hold.

My guess is the puppet masters tell them how to pose for the photo, and not all of these people are part of it. Then again, they may not get the opportunity unless already part of it. So I can see either possibility.
