Tennessee just did what Congress can't. They passed a law to break up the health insurance giants. Specifically, they made it illegal for pharmacy benefit managers — the companies in charge of pharmacy insurance — and pharmacies to be owned by the same company. That makes perfect sense. For example: CVS Caremark is the PBM, and CVS is the pharmacy. So if you have Aetna insurance, you have CVS Caremark as your PBM, and they're going to do everything they can to make sure you use CVS as your pharmacy. Aetna, Caremark, CVS — all the same company. That causes all kinds of incredibly obvious problems that this law hopes to fix. If your insurance company is in charge of approving your medication, deciding how much to pay for it, AND deciding who gets that money — while also being the pharmacy that gets paid at the end — guess what happens to prices? They go up. Governor Lee signed the law last week. CVS immediately filed a federal lawsuit because they said it will force them to close all 136 stores they have in Tennessee. Let that sink in. I'm not sure most people realize what that says about CVS and health insurance in general. They had to choose between owning the middleman (the PBM) or the healthcare provider (the pharmacy). Without hesitation, they chose the middleman. The biggest pharmacy chain in the country — with a store on every corner — would drop all 136 of their Tennessee locations in a second if it means keeping their middleman business. It is more profitable for them to be a health insurance middleman getting between you and your healthcare than it is to actually provide the healthcare. That is the problem with healthcare in America. We have made the middleman so powerful that they've taken complete control of the entire system. Three PBMs — Caremark, Express Scripts, and OptumRx — handle around 80% of all prescriptions in this country. How on earth can we expect healthcare to work well and remain affordable if that's where the money is? We all auto-pay our insurance straight out of our paycheck before we even see the money. And not surprisingly, they're keeping a ton of it. That's why we fired them. And they can't file a lawsuit to stop us. That lets us offer fair, transparent prices. No PBMs. No insurance games. No hidden markups. You see the cost, you pay the cost.

BIG NEWS: Oklahoma Attorney General’s Office has filed a lawsuit against CVS Caremark for Pharmacy Benefits Managers violations This is such a widespread and massive problem, Oklahoma had to create a new court - CVS Caremark PBM is denying covered medications - They are paying independent pharmacies less than the cost of medication (in many cases PAYING $0, “It's not uncommon for covered medications to pay literally $0 to the pharmacy” - The reimbursement that comes back to independent pharmacies is totally unrelated to the cost of the medication - “CVS still has the audacity to take your absurdly large premium and not use that to pay the healthcare providers you need to actually care for your health” (They literally pocket the money and pay independent pharmacies next to nothing, or NOTHING) “That's what the Oklahoma suit is about. They have a law that states the PBMs like CVS Caremark aren't allowed to pay less than the cost. Of course, that didn't stop them” “This case brings 200 prescriptions that were reimbursed under cost and denied on appeal by CVS. I am not so partisan that I can't appreciate Congresswoman Alexandria Ocasio-Cortez taking down the CEO of CVS on behalf of all Americans. Healthcare is a universal issue, so pay attention to what's being sold to us. Translation: "Our perfect patient is insured by Aetna, CVS. They are seen at Oak Street Health, CVS. The prescriptions and drugs they take are negotiated in price by Caremark, CVS, and they pick it up at, oh, you guessed it, CVS." This one-stop shop idea doesn't benefit the end user; it benefits the companies selling it to us. This is not healthcare; it's herding cattle. We all need to get behind this reform.

American Doctor doesn’t know what else to do A health insurance company cancelled her patients heart failure medication and keeps denying her prior authorizations “I f*cking can't today, I wasted an hour and 17 minutes of my time on the phone with an insurance company who decided that my patient no longer needs their heart failure medicine that they've been on for years. That is an old medicine, not remotely experimental. It's what everybody uses for heart failure patients in this situation. It's a medicine that saves the insurance company money in that if they pay for 36 patients to use it for three and a half years each, they'll prevent one heart failure exacerbation according to the number needed to treat for all of you methodology geeks out there. And you know, the total cost of all that medicine for 36 patients for 3 1/2 years is less than the cost of like, the first hour of a heart failure exacerbation hospitalization. So this makes no sense. — To go through that bullshit website and put in the information just to be told there's a duplicate prior authorization? I'm like, well, I'm the physician that wrote the prescription, so how about we delete that duplicate because it doesn't matter. And they're like, well, we can't do that. Why don't you talk to the insurance company? I'm like, sh*t, why didn't I think of that? Get back on the phone. Cannot get a single person on the phone for the insurance company. The phone tree directed me to the nurse advice line which patients are supposed to call to find out do I need to go in for my sore throat 3 times. And then I got forwarded to Express scripts. Totally different company. Not even my patient's pharmacy. How does that happen? My patient still doesn't have their meds. If you're wondering why you can never see your doctor, it's because your doctor's doing this bullsh*t This is a major contributor to burnout in primary care. — This makes no sense for anybody. And my patient still doesn't have their meds.”

I know from watching Patrick’s testimony about his route to get the takeout that night that he did take the fastest route to the 3V. I take the same route when I go back and forth from my house to the CVS in Kingston. I live very close to the 3V. I know from watching Patrick’s testimony about his route to get the takeout that night that he did take the fastest route to the 3V. I take the same route when I go back and forth from my house to the CVS in Kingston. I live very close to the 3V. One the BIGGEST problem/Harassment/Loot which patients and their families faces in Hospitals is That they are forced to buy medicines from the in house medical within the Hospital which in majority cases charges very very high prices compared to same medicine in market. There should be Law to ban this practice and let patients buy medicine from market. In many cases you never if the medicine was administered to patient or is it routed by the in house medical store to be sold again to another patient. what is your experience on this practice?

I just found out that Luel AI, the company that completely copied my company Kled's entire website, is misrepresenting their compliance practices, using Delve to outsource it, fabricating their website user numbers, and pulling in massive amounts of fraud data from exclusively fraud oriented regions. Skip to 1:15 if you want to skip context. You truly cannot make this up. This is egregious, disgusting behavior from everyone involved. This is people's data. It cannot be taken lightly. The only way a company like this can truly succeed is by investing the time and resources to commit 100% to the trust and compliance of their users' data. Luel AI is doing the exact opposite. Disgusting behavior for a company trusted with people's data.

Anti competitive practices are harming patients in Austin right now and I am asking Congress for help. Insurance companies decide where my patients are permitted to have surgery. They have refused to recognize my surgery center @redbudsurgerycenter as in network, even though it is fully certified by CMS, meets all safety standards, and is often a safer environment for complex cancer surgeries. My ask to Congress is simple. Allow doctors and patients to choose where surgery is performed. Pre existing arrangements that force care into specific facilities are anti competitive and not in the best interest of patients. This week my breast cancer patients needed surgery and were unable to receive care because of insurance. Cancer does not wait. If we want to talk about cracking down on insurance company practices, this is the place to start. What you can do right now: - Call your insurance company and ask for Redbud Surgery Center to be considered in network so we can take care of our patients. -Call your member of Congress and ask them to require insurance companies to allow doctors and patients to choose the facility where care is delivered, especially when patient safety is at risk.
