Kitten learning how to use the litter box

The kitten is in discomfort and sick. It has diarrhea can’t you see it in the litter? And is constantly trying to release itself by trying different spots. It’s need to be taken to the vet and given meds to help. I had cats before I’ve seen this behaviour.
