NTSB has published a docket for the Medivac Learjet that crashed in January 2025, no final report yet, not many clues. But their performance analysis agrees with my reconstruction, showing a steady rollover possibly due to spatial disorientation.

And they basically have ruled out everything else. Acoustic analysis from Honeywell shows both engines were at 10,900rpm right before crash, corresponding to takeoff/climb thrust, so no engine failure; pitch trimming actuator was tested with no mechnical failures found.

But that pitch trim actuator disassembly result is problematic. Elevator alone is very ineffective against that flying horizontal stab in my Lear experience. Not sure what is up with that trim position.

My daughter is learning to fly, and spatial disorientation is talked about a lot. Oddly or not, my Gen Z daughter is all about analog or "steam gauges," as she calls them. She said, "Give me steam gauges in a ' 6-pack ' over a glass screen any day."

Does "spatial disorientation" mean they couldn't tell which way was up? I've seen incident analysis for oil rig transport choppers. This issue was sometimes the case when flying over open water at night. The pilots flew into the sea despite audible "ground proximity" warnings

Yep, couldn’t see in the clouds, was distracted, missed the instruments, didn’t feel the plane was slowly rolling over.
