After initiating androgen deprivation therapy, we performed HoLEP before definitive radiotherapy. Despite the advanced disease, the procedure was remarkably straightforward: minimal bleeding, excellent surgical planes, no technical challenges beyond standard HoLEP, and successful spontaneous voiding just 36 hours after catheter removal.
