Good reminder for PTs. Joint mobilization isn’t simply “push the joint this direction and gain that motion.” A posterior glide may help improve shoulder internal rotation in some patients, but the response is individual. Pain modulation, reduced guarding and sensory input to the nervous system can all change how someone moves immediately afterward. Assess the restriction, treat what you find, then reassess. The patient’s response should guide the treatment, not just a chart of mobilization rules.

The pain-modulation point is easy to miss. Sometimes range changes right away, but the real win is making that new range feel comfortable when you load it afterward.

Love the reminder that there isn’t one perfect technique—meeting people where they are and adjusting is such a healthy approach.
