Dr. Abhishek Shukla, Geriatrician, Explains: Why Did a Patient Develop UTI Symptoms 14 Times Despite Repeated Treatment? A 56-year-old man presented with recurrent urinary symptoms despite receiving treatment on 14 separate occasions. He experienced temporary relief after taking medication for four to five days, but symptoms returned within approximately ten days. His complaints included burning during urination, frequent urination, and a weak or interrupted urinary stream. Routine urine examinations sometimes revealed pus cells, while urine cultures occasionally identified bacteria. This raises an important question: Are we missing an underlying condition? Dr. Abhishek Shukla explains that repeated urinary symptoms should not always be treated as uncomplicated urinary tract infections (UTIs). If symptoms persist despite appropriate antibiotics, the underlying cause must be investigated. One possibility is bladder pain syndrome, also called interstitial cystitis, which may cause persistent bladder discomfort, urinary urgency, frequency, and nighttime urination, often without an identifiable bacterial infection. Other important differential diagnoses include bacterial prostatitis, benign prostatic hyperplasia (BPH), incomplete bladder emptying, bladder stones, urethral stricture, overactive bladder, and, in selected cases, bladder malignancy or genitourinary tuberculosis. The next step is to review previous urine culture reports, obtain urine cultures before antibiotics when appropriate, assess prostate health, and investigate urinary obstruction or residual urine. Depending on clinical findings, ultrasound, uroflowmetry, cystoscopy, or specialized infection testing may be required. The key message: Repeated antibiotic courses without identifying the cause may delay the correct diagnosis and contribute to antibiotic resistance. Recurrent symptoms require careful evaluation rather than automatically prescribing another antibiotic. Dr. Shukla recommends further investigation and urological consultation to establish the diagnosis and provide appropriate treatment. #UTI #RecurrentUTI #UrinaryTractInfection #BladderPainSyndrome #InterstitialCystitis #AntibioticResistance #UrineCulture #Prostatitis #BenignProstaticHyperplasia #UrinaryRetention #Urology #BladderHealth #GeriatricMedicine #DrAbhishekShukla #MedicalEducation
