Scratch test for organomegaly. First do a "control". Won't hear scratching until very close to scope. Next put scope over organ (eg, spleen). Now you'll hear scratching when you reach edge of the organ (much farther from scope vs control). The organ "carries" the sound to scope.

As a doctor In acountry where you rely mostly on your clinical abilities rather than inmediately taking US/XRAY/CT, I do find this useful. As a pulmonology resident, we get used to exploring since day 1, based on the same fact we may (or may not) have an image soon, so THANKS!

@HaryaxPathak Pretty useful. Nearly 20 years ago, I picked up splenomegaly in a patient with diffuse systemic sclerosis and anaemia in the exam (using this) and stumped the examiners who had missed it. Very useful in not missing soft liver or spleen, when your life depends on it.

According to this article, the technique has 90% specificity (10% false positive rate) when evaluating the liver. Not sure about sensitivity but would guess lower.

Some organ donors in America are not truly brain dead when the organ procurement process begins, says Dr. Joseph Varon, co-founder of Independent Medical Alliance. “I have seen multiple occasions where a person supposedly was brain dead. If you are brain dead, you don't need any permission from anybody to disconnect somebody from a machine…but there are specific criteria as to what constitutes a patient that is brain dead. So a brain dead person, for example, would not have any feeling. When I was doing my fellowship training…I would fly with a cataract surgeon to wherever was the dead person. So we would examine the lungs, and if the lungs were good, we would get the lungs out. In order to do that, I would do a procedure called bronchoscopy, where you put in a scope into the lungs, and if you are dead, you don't cough, you don't move, you don't do anything. There was more than one instance where I would go in, I put in my little scope, and the patient starts either moving or coughing. And you look at the nursing staff and you say, ‘Hey, this person is not dead.’”

A charity cancelled a choir because its leader believes that women do not have penises. Scope, a charity which supports disabled people, had invited a community choir to cheer on runners at the London Marathon under its banner. But just days before the marathon, Scope rescinded the invitation because the choir leader, Janet Murray, has publicly stated that she believes women are biologically female and that women should be entitled to single-sex spaces. Scope said these views conflicted with its “commitment to diversity and inclusion”. In correspondence with Janet, the charity reportedly said: “We are concerned about your views because we don’t agree with your views.” It seems Scope is more concerned about policing lawful opinions than supporting its charitable work on behalf of disabled people — many of whom rely on single-sex care and spaces themselves. Cases involving people being penalised for gender-critical beliefs remain one of the largest categories of cases handled by the Free Speech Union. Under the Equality Act, gender-critical beliefs are protected philosophical beliefs. Janet is a member of the Free Speech Union and is taking legal action against Scope alleging unlawful discrimination on the basis of her protected beliefs. Our new organ has had a profound impact on congregational singing in Chapel and on the development of organ studies at Radley. 🙏 For Kim (6.2, K) and Bernardo (6.2, H) the new instrument, built by Nicholson & Co. Ltd of Malvern, inspired them to take up the organ. They have both recently received offers to become organ scholars at Cambridge University; Kim to read music at Selwyn College (after a gap year at Portsmouth Cathedral ☩) and Bernardo to read maths at Trinity Hall. They will join Henry Morris (2017, H) who is currently organ scholar at Magdalen College. We hear from them all in this short film, as well as Dr Tim Morris (Timothy Morris). Tim tells us about the opportunities for organists at Radley, the ‘tricky’ design brief and how the boys’ favourite hymns have never sounded better. RCO
