Should be four. I remember Jonathan Trott doing that in a Test match.

I can't remember but I think the law is that if the ball just slips out of the hand in the run up it's a dead ball, but if it comes out like that when the bowler bowls it overarm, it can be given no ball and hit. But that would make this one quite ambiguous

Looks to have come out before he reached the delivery stride. I think because of that it should be a dead ball. If the ball came out funny and did that as his arm came over to deliver it then I think it's live and the batsman can smash it like that.
