The Board of REACT19 regrets to inform our community of the sudden cancellation of this week’s meeting of the Advisory Committee on Immunization Practices (ACIP) at the CDC. The meeting was cancelled following a federal court ruling that blocked recent vaccine policy changes and invalidated the current ACIP advisory panel while legal challenges move forward. For many in our community, this meeting represented a long-awaited opportunity. After more than five years of waiting, patients and families had prepared impact statements in hopes that the urgent need for clinical recognition, research, and care for those suffering from serious post-vaccination conditions would finally be discussed. For those living with these conditions, this cancellation is deeply painful. It means more waiting, more uncertainty, and more time without the clinical guidance needed to help physicians recognize, diagnose, and treat patients who have been suffering for years. Our hearts are with every member of this community who had hoped their voice would finally be heard this week. However, even in this difficult moment, important progress continues. On March 18, REACT19 will still be part of a historic step forward at the CDC. A proposal for a new ICD-10 diagnostic code related to post-COVID-19 vaccination conditions will be presented, opening a public comment period and moving the medical system closer to formally recognizing these patients. Diagnostic codes are more than administrative tools. They are a critical foundation for clinical recognition, medical education, research, insurance coverage, and ultimately the development of treatments. Establishing an ICD-10 code would represent an important step toward building the clinical infrastructure needed to care for patients who have too often been left without answers. How can a 99% accurate medical test give you a 9% chance of having the disease if it comes back positive? 🤔 If you are in medicine this is the SINGLE most important diagnostic testing concept to know. Welcome to the difference between specificity and positive predictive value. Sensitivity & specificity are fixed test properties. These do not factor how common a disease is (prevalence) Positive Predictive Value (probability a positive test reflects having a disease) factors in prevalence and is actually more important to clinicians than sens/spec. It is harder to figure out though because we need to have a gestault for how prevalent a disease is for the EXACT patient we are seeing. If you have very low prevalence, even with a great test, most positives are false positives. This is why screening low-risk patients can result in many false positives and harm To master this, just play with the calculator yourself and you will see!!!!👇

Last week I did spot checks at Masaka Regional Referral Hospital after hearing from the public and whistleblowers. Yesterday I met with the hospital administration and we agreed on the following; 1. Accountability & Monitoring: Deploy more hospital inspectors and staff to the control room to monitor CCTV cameras. Take firm action against absenteeism, extortion of money from patients, and unprofessional conduct by some health workers. 2. No Excuses: The administration informed me that there has been a recent salary enhancement and more recruitment. Therefore, there is no justification for absenteeism without valid reason, extortion, or unprofessionalism. 3. Oversight: I committed to visiting the hospital once every month to monitor progress together with the administration. Spot checks will also continue based on reports from whistleblowers. 4. Service Delivery: We discussed the delayed payment of interns and the need for Government to make scan services completely free, since most people cannot afford them. I also raised complaints from Banna Masaka that some medical workers, especially nurses, are rude to patients. The administration promised to continue counseling staff to be more compassionate and patient-centered. 5. Progress: With the new complex now operational, no mothers are sleeping on the floor anymore and our NICU is fully equipped. 6. Fighting Fraud: I ordered the arrest of a conman who has been defrauding patients. The administration claims some people keep him in the hospital, saying he is an "NRM cadre". It is unacceptable to use NRM or any political party as a cover to steal from patients. I also committed to lobbying Government for more infrastructure and drugs for the hospital. Imagine qualifying as a doctor in the UK, only to be told there is no NHS training job for you in your home country. That is the reality facing thousands of UK medical graduates. The taxpayer invests in medical education. Medical graduates complete a five to six year demanding medical degree, graduate with debts often exceeding £100,000, and commit themselves to serving patients in the NHS. Yet in 2025, more than 40,000 applicants applied for just 10,000 specialty training jobs. Some doctors end up unemployed because they cannot find jobs as fully qualified doctors. These training posts are not optional extras. They are the route to becoming the specialist Consultants and GPs our NHS depends on. Without them, doctors cannot progress in their careers and patients ultimately lose future specialists. Yet UK medical graduates are not being prioritised for these NHS training jobs. Prioritising UK medical graduates for speciality training jobs is coherent workforce planning. At the same time, there must be safeguards for international doctors with significant NHS experience who are already contributing to patient care. Right now, the system satisfies no one. It creates unemployment, fuels insecurity, and weakens the future medical workforce. That is not sustainable and it must change.

A Pap smear is a screening test that takes a sample of cells from the cervix. The aim is to detect cellular changes before they turn into cancer. My physical therapist opened a superb gym in a beautiful and intimate space in a great location at 32 East 57th Street next to Atria. The concept integrates physical therapy with weight training, cardio, sauna, steam, cold plunge, sensory deprivation, VO2 max and all the other testing you need to determine your base line fitness and monitor your progress thereafter as well as much more. Vosk has the best equipment I have ever seen in any gym and extremely experienced trainers and physical therapists. It is a great place for serious athletes and those interested in longevity or just getting in great shape. While it is a high end gym, the price is fair when you consider the cost of the 96 training and/or physical therapy sessions included in your annual membership. The integration of physical therapy and training yields much better and safer outcomes. I have also found my training consistency is much greater since I prepaid for the sessions. Vosk has the best equipment I have ever seen in any gym and extremely experienced trainers and physical therapists. It is a great place for serious athletes and those interested in longevity or just getting in great shape. While it is a high end gym, the price is fair when you consider the cost of the 96 training and/or physical therapy sessions included in your annual membership. The integration of physical therapy and training yields much better and safer outcomes. I have also found my training consistency is much greater since I prepaid for the sessions.
