James can get a nervous tic if he gets too stressed. It can only trigger after he enters Brookhaven, the hospital likely triggers a PTSD response and the stress remains afterwards. Interacting with a red square stops the tic. Perhaps what helps is the feeling of "someone groping around his skull" that he mentions in the opening..? As though something "resets" his nervous system. James looks more tired, he has a different animation when he runs out of breath, the inventory is different, some melee animations are different, metallic sounds sometimes play and make the controller vibrate as though Pyramid Head is nearby, and there's some unused soundtrack. Also Mary's letter and photo aren't in the inventory and Maria doesn't appear at all. Wait until the end for James vibing to an unreleased track! The track plays immediately when starting the demo and I thought that's very disturbing... #silenthill #silenthill2

It also looks very similar to the view on a mental health poster.
