There's a class of drugs called PCSK9 inhibitors that few people have heard of but Alex thinks will be far bigger than GLP-1s (which is already one of the best selling drugs of all time). They were discovered through a population born with a genetic mutation that gives them an 88% lifetime reduction in cardiovascular disease risk. The drugs mimic that mutation, significantly lowering our bad cholesterol and reducing the risk of a future heart attack or stroke. Alex describes them as close to a free lunch as there is in biology. "Because it is more asymmetric, it is more of a free lunch, I would argue that this medicine should be far bigger in terms of the number of people that are on it and the revenues associated with it than GLP-1s."

Nestle says their company is looking to increase flavors in their products “to counter duller taste buds that can accompany GLP-1 weight loss drug” Nestle and the media is saying they will do things like “adding spices or more pepper” but that’s not at all what they really plan on doing I’ve made posts about this and what they are really doing is engineering foods to be more addictive to counter the GLP-1 drugs There's a food laboratory in Northern California called Mattson that is quote unquote creating the future of food. Mattson has conceptualized, developed, branded, and scaled hundreds of product lines, and in the process created billions of dollars in revenue for clients Right now their Big Food clients are asking them to do experiments that will reformulate ingredients so that their food can bypass the effects of Ozempic, wegovy and other GLP-1 weight loss drugs They’re engineering the food to be more addicting This is what I believe is really going on. There’s no doubt these drugs are incredibly effective for weight loss. But the high doses currently being used might be too much of a good thing. For example, high-dose GLP-1 receptor agonist use has been linked with a near doubling in the risk of suicidal behavior and a tripling in the risk of depression, as well as a two-fold increase in the risk of age-related macular degeneration. However, these drugs may still have utility when used strategically. According to Dr. Ben Bikman (Benjamin Bikman), micro-dosing GLP-1 receptor agonists intermittently may be a beneficial strategy to manage hunger, reset eating behaviors, and encourage healthier dietary patterns. Specifically, Bikman proposes that using low doses for about 90 days could fundamentally shift one's relationship with food. This woman is saying what a lot of people need to hear. Her weight gain didn’t come out of nowhere. Mental health struggles, pouring everything into her job, and a broken relationship with food and movement all came first. She’s now treating her ADHD, working on her mindset around eating, and moving more consistently. GLP-1 meds might reduce her weight, but they don’t touch what’s really driving it: how she thinks, her mental health, and her relationship with food and herself. She’s not willing to risk her current stability just to make other people more comfortable with how she looks. The real fix isn’t just shrinking the body. It’s addressing what’s going on underneath. She’s pointing out that GLP-1s can change the weight but doesn't fix the mental and emotional stuff underneath. Do you agree with her?

It's surprisingly plausible that we got GLP-1 drugs because of Danish tax law. For tax reasons, Novo Nordisk is actually controlled by a non-profit foundation. There are many downsides of this. But one possible upside is that it's helped the company avoid institutional drift. The founding goals of the company were written into the non-profit's constitution, so now every new generation of leaders has to abide by them. In particular, the company is legally mandated to reinvest significant sums in R&D, regardless of the effect on shareholder value. That R&D gave us one of the major drug breakthroughs of this century so far.

When people say young Americans aren’t lazy, I point to reports like this. According to a survey from healthcare platform Tebra, 1 in 4 Americans, and 37% of Gen Z, are planning on turning to GLP-1 weight loss medications to lose weight. Instead of traditional methods like exercise and eating right, many Americans aren’t choosing to take the easier option. “Women plan to lose 23 pounds on average in 2025, while men are aiming for 19 pounds,” Tebra revealed. The number one reason people said they were turning to the medication was because they “want faster results.” Half of the people polled said they gave up their health goals in 2024 with 31% giving up by February.
