The response to yesterday’s post about opening KaufCare has been incredible. After 20 years in the ER I saw the same thing every shift: Patients waiting forever. Ridiculous bills. Frustrating for patients & docs. Too many middlemen. That’s why we’re trying a different model.

I’m not patting myself on the back, but I’ve been in the doctor business forever and when they passed the Hitech act in 2009 I knew it was a pending disaster, very few listened to me. This made it mandatory in EMRs that patients could email docs, and the doc had to have capacity to answer. This sounds wonderful and innocuous, no one but me saw the future. This feature, electronic messages from patients, is an absolute disaster for docs and NPs, I bring up NPs because they are the ones stuck with this, the docs dump it on them. Many docs admit they only hire NPs to handle these, and to handle crazy, needy patients. I can teach docs and NPs how to handle these and reclaim their lives, but again, they won’t listen to me, they’d rather burn out and quit the profession entirely. If you’re a patient reading this and think it doesn’t matter to you, you are “dead” wrong

I genuinely want to know why government hospitals in India are in such terrible shape. Why is no one held accountable for the lack of basic facilities? I saw another post where patients were sitting on the floor, & a dog came & urinated on the wall. How is all of this acceptable?

James Harden said he’s been on teams where there were “individual agendas” and this Clippers team doesn’t have that. “All of us are on the same page. We aren’t sugar coating anything. There’s no hidden agendas. We’re all trying to accomplish the same thing… Been in situations, certain teams where guys got individual agendas — whether it’s contract years or trying to make a name for themselves… Now for the most part, the 11th guy we all got the experience… It’s about one thing. And I think that’s a great thing going into the season.”

He has spinal nerve compression, he has severe weakness, and he has a collapsed neck. So these are the challenges that we have every day. Our team has two incredible, full-time professionals—Mary and Ebonie—dedicated to dealing with insurance. They advocate for our patients, so our patients can focus on healing. This week? They’ve been on repeat calls with the same insurer, trying to add procedure codes so patients can receive coverage for sensation preserving mastectomies. Twice now, after 30+ minutes on the line, the call just ends. No reference number. No resolution. Just—terminated. And without a reference number, there’s no way to track it, no accountability, and no clear path forward. All we have are the first names of the representatives who hung up. This happens often. And it’s exhausting. But we won’t stop showing up for patients. We’ll keep sharing these stories—because this shouldn’t be normal. Patients deserve better. Providers deserve better.

As a resident doctor in England, I’ll be back on strike tomorrow - and I want to be clear about why. We have NHS doctors who want to serve patients, who trained for years, who are being pushed into unemployment on top of Government caps on how many doctors can train to become Consultants or GPs. That means fewer doctors for your A&E, your GP surgery, your hospital wards. At the same time, Wes Streeting is recommending a real terms pay cut next year, on top of the 20% pay erosion we’ve already taken. It’s getting harder to keep doctors in the NHS; and patients feel that every day through longer waits. We’re striking because the current system fails patients and leaves skilled NHS doctors on the sidelines. Wes Streeting could end the strikes right now by putting forward a credible offer. Until then, I’ll be on the picket lines with Resident Doctors; because standing up for doctors is standing up for the public too. #payrestoration #endtrainingcrisis
