You are never committed to the takeoff until you are beyond V1. And the decision making process revolves around correctly identifying the failure and making your go/ reject decision by V1.🤷‍♂️

V1 is the decision speed—the last speed at which a rejected takeoff is guaranteed to stop within the available runway. Above V1, the takeoff must be continued unless the aircraft is clearly unable to fly. But I read somewhere that when calculating takeoff performance (for any eventuality on the takeoff roll, like engine failure), instead of continuing the takeoff, the pilot flying would wait 2 seconds after reaching V1, then decide to reject the takeoff and safely come to a stop on the available runway distance. Someone just received an invitation to see the Chief Pilot, and it's definitely not for tea and biscuits. Malaysia Airlines A330 (9M-MTI) narrowly avoided serious nose gear damage before takeoff. On December 18, Malaysia Airlines Airbus A330-300 9M-MTI, operating flight MH71 from Tokyo Narita to Kuala Lumpur, was cleared for immediate takeoff from Runway 34L in heavy rain. During the initial takeoff roll, the aircraft drifted left, and the crew rejected the takeoff at around 30 kt ground speed. After slowing to about 8 kt, directional control was regained—but instead of vacating the runway, the crew continued the takeoff roll and departed safely. Another ill-fated take-off over V1 (in this case, V2!) June 13 1996: Garuda Indonesia Flight 865, a DC-10, crashes in Fukuoka (Japan) 3 of 275 aboard die. On take-off, the jet had just left the ground when one of the engines failed. Crew opted to reject the maneuver; the aircraft was unable to stop safely and left the runway: landing gear collapsed, and the airplane caught fire. Inquiry noted pilot actions by aborting takeoff above V1, where SOP dictated pilots should have continued takeoff and deal with the engine later. The nose was raised, and at a speed of 158 kts, the first officer called "Rotate". It was 12:07:40. At 12:07:45, the flight engineer called "Engine failure number one." The thrust reversers were deployed and ground spoilers were extended. The DC-10 skidded off the runway through a ditch, fence and a road, before coming to a halt 620 m past the runway threshold. Cathay Pacific A330 aborts its takeoff in Kaohsiung after a loud bang and what appeared to be flames from one of the aircraft’s engines. A video posted on Facebook of the incident shows the airplane suddenly decelerate after a bang is heard and a flash of flame is seen coming out of one of its engines. The aircraft veers off course slightly before correcting and safely taxiing back to the terminal. A Cathay Pacific spokesperson said CX431 was aborted “due to an engine technical problem”. The spokesperson apologized for the inconvenience that may have been caused by the incident, and said safety is a priority for the airline.

The more you watch it, the worse it gets. The fact that they didn’t reject the takeoff is insane. You can clearly see the flaps moving during the takeoff roll — a big no-no. A Takeoff Configuration Warning System (TOCWS) activation should trigger an immediate rejected takeoff (RTO), followed by getting the aircraft off the runway and checking that nothing else has been missed. Not sure which airline this is, but something like this can be a career terminator. The local CAA is going to have a field day with this one, to say the least — that is, if it’s reported. Oh, and I almost forgot: the cabin crew’s “take your positions” call came just as the aircraft was entering the active runway. How long do they have before they’re actually seated? 📹: Normal-Compote6294

Avianca flight AV9418 evacuated after an aborted takeoff from Pereira Matecana International Airport in Colombia. The Airbus A320, with 168 passengers, was flying from Pereira to Bogotá. However, on departure there was a problem with one of the engines that prevented it from taking off. "According to preliminary information, an A320 aircraft that was starting a flight between Pereira and Bogotá had to suspend takeoff on the runway, apparently due to a failure in one of its engines," the Civil Aeronautics department said.
