Constipation is not an occasional inconvenience with these drugs. It is a direct and expected result of how they affect the digestive system, and when it goes far enough it has caused catastrophic damage. Tom Segura and Bert Kreischer get into this exact problem on the podcast, talking about the multi-day shutdown that comes with pain pills and the case of Matthew Perry, whose colon ruptured after prolonged use left him unable to move his bowels. The wider conversation around opioids tends to focus on addiction and overdose risk. The mechanical failure of the gut gets far less airtime, even though medical literature has documented opioid-induced constipation as both extremely common and capable of progressing to impaction, obstruction, or perforation. The body simply stops doing one of its most basic jobs. Any substance that disables a fundamental biological function this consistently deserves to be weighed with that cost in full view.
