The official SH2 novel explains James' thoughts when he sees the noose. In SH2R it's obvious a lot is going on in his head, but the details are left out. In the novel he imagines his execution. Pyramid Head pulls the noose around his neck, then James realizes he's trying to hang himself and snaps out of his delusions. At that moment James is so mentally disturbed and desperate to escape his nightmare that he calls out to the Brookhaven director for help, believing he's his patient: “Help me...Somebody please...help me... If you’re really there, looking into the face of your deranged patient, and if I’m really there, too, then please help me. Use medication or whatever you want, I don’t care how rough it is. If you disable music, you'll hear a man speak to James over the prison's speakers. I can hear "you've killed somebody", but can't make out the rest. He sounds exactly like the Brookhaven director, so I assume it's him. It's possible he's watching James throughout the game, it's hinted a few times in SH2R and implied strongly in the official SH2 novel. I don't understand why in SH2R James can't attack nor kill Maria. It served as a subtle hint that James is capable of killing someone he loves, and attacking her with the chainsaw made her scream just like the monsters, hinting at her true nature. Instead, in SH2R James refuses to aim at her, and if the player tries to aim at her she simply complains about the flashlight. While attacking Maria may appear as a useless mechanic, knowing the full story gives it a whole new meaning, and I wish it wasn't left out of the remake. Perhaps they could have added just a little hint that it crosses James' mind for a moment to hurt Maria. #silenthill #silenthill2

It also looks very similar to the view on a mental health poster.
