I subscribe to the idea that Tony didn't actually see Big Pussy, he just sensed him or at the very least could tell that something was...off. We the audience see Big Pussy, because the Sopranos is a television show, and the best way to convey this scene to the audience is to show Big Pussy in the mirror. If you had Tony just looking and acting uneasy while Paulie and Furio talked about something unrelated, then the scene would seem pointless and overly cryptic

Pay attention to Butch in this scene. He seems open to the idea of going to war at first, but he still needs Phil to sell him on the reasons for it. Phil can’t do that. He blows it. Butch even tries to get the ball rolling by talking business to Angelo about how New Jersey’s “redundant upper management” is decreasing their share of the profits. Phil cuts him off soon afterward because that is irrelevant to him. This has nothing to do with business or the bottom line. It is about Phil’s damaged ego. He disregards the deaths of Coco, Fat Dom Gamiello, and even his own little brother Billy as reasons for going to war. Instead, he focuses on Tony never having served time in prison like he did. Why is that so important? Because it reveals Phil’s true motives. This war isn’t about business or Mafia rituals. It is about Phil’s bitter resentment toward anyone who didn’t have to sacrifice one-third of their life to prison like he did. It also shows why Phil could never have won this war under any circumstances. He was taking his family to war while making the entire cause about himself. He dragged everyone into an unpopular war for vague, personal reasons—reasons that could never be resolved later at the negotiating table, the way they were supposed to be. Phil believes his 20-year prison sentence gives him free rein to do whatever the hell he wants now that he is boss. Phil is a horrible Mafia Don. He is uninspiring, snarky, and increasingly aloof, and he has convinced himself that every mobster in New York owes him their life because he served his full sentence without ratting anyone out to the FBI for a lighter sentence when he easily could have. No matter how often he reminds everyone that he “did his time like a man,” Phil now seems to secretly hate himself for it. He regrets having stood up for everyone else in the mob after seeing how much the New York Mafia had eroded during the two decades he spent in prison. That is where Phil loses Butch, Little Carmine, and eventually the support of his entire family for this unnecessary war. He made it all about himself, not about business or anything else concerning the rules of war. He ultimately loses everything because he turns the conflict into a matter of personal vanity while gambling with the lives of other men. Phil has convinced himself that everyone owes him their lives because he stayed silent for 20 years. It is a distorted god complex that Phil has been hiding for years, but it is finally revealed in full here. With Phil, his personal grievances will always matter more than the lives of the men he is supposed to lead. Phil Leotardo is the epitome of a terrible boss.
