Dr. Caleb Masterson say health insurance companies are openly breaking laws in Alabama He explains some of his patients situations with insurance companies - “I had an insurance company tell a patient that no authorization for the procedure was required, the procedure was performed, and then the patient was denied the care because the procedure was never authorized. So the patient got charged the entire cost of the procedure” - “I had another patient who is supposed to be due an answer to an insurance claim within 30 days of submission. If they do not reply within 30 days, that's breaking Alabama law. And that claim is currently at the 65th day, so its insurance can be breaking the law.” - “Then I had an insurance company deny care to a patient that they had previously approved. So a patient qualified for surgery based on medical necessity, they had met all the criteria for surgery, they underwent surgery, then the patient had that surgery denied. The patient submitted that information to the insurance company and said, Hey, look, this was already pre-approved. And they said, We reversed our decision and the patient had to pay.”

What is the meaning of antimicrobial resistance? It just means that when you develop a serious infection, none of the antibiotics will be able to treat your problem. Why did we land in this trouble? I have been practising heart surgery in India for the last 36 years. The first 15 years were the best period of my life. After any heart operation, however major it was, patients had antibiotics just for two days, and then we stopped the antibiotics. For whatever reason, if the patient's condition deteriorated after the surgery, sepsis or infection was never a thought we had in our mind because infection just wasn't there. In the last 10 years, things have dramatically changed. Today, our greatest fear when we do complex heart surgery or any surgery for that matter is infection. The problem is that infection can happen, but most of the common antibiotics we use today, they just simply do not kill the bacteria. This happened because of the rampant abuse of antibiotics. All these bacteria have eaten so much of the antibiotics that today they don't care for the antibiotics..."

2026: A patient reached out to me yesterday to let me know about a concerning interaction with a hospital system. The patient was scheduled for a therapeutic procedure, in advance of surgery. Arrangements were made and the patient paid their estimated portion of the cost. Then they received a call from the facility asking them whether they still had the same insurance. When the patient confirmed that nothing had changed, they received a warning from the facility that the estimate no longer applied and costs could go up…significantly. Why? Because the hospital system is renegotiating with the insurance company. Not because the clinical situation changed, not because the procedure became more complex…just because the patient is caught in the middle of a business negotiation without an advocate. So they called me. And I’m telling all of you. So we can help one another by insisting that patients come first. Always.

My last case of 2025 and my first case of 2026 tell you everything you need to know about our healthcare system. Last week, I was scheduled to do a DIEP flap for a patient who had been waiting a long time. We had asked her insurance company if we could do the surgery in network at @Redbudsurgerycenter, where we have everything needed to safely and efficiently take care of her. They hadn’t approved that, so the surgery had to be scheduled at an older hospital in town. When we arrived, the hospital was having HVAC issues. The humidity levels were not safe for surgery. We waited for hours, and ultimately the case had to be canceled. It wasn’t just her surgery. Multiple surgeries were canceled that day because operating under those conditions would have been a fire risk. We weren’t able to reschedule her surgery before the end of the year, which means her deductible resets. I could have safely done her surgery at Redbud that day but insurance rules wouldn’t allow it. This is what closed networks and rigid systems look like in real life. Now here’s the contrast. The first case I’m doing in 2026 is also a woman who needs breast reconstruction with DIEP flaps. This time, instead of traditional insurance, her employer uses a third party administrator. They came to me directly and asked what I would charge if they paid fairly and transparently. On Monday, we’re doing her bilateral DIEP flap at @Redbudsurgerycenter. I can control the environment, the costs, and the care. It’s better for the patient and significantly less expensive for the employer. This is called Direct Specialty Care. It’s not direct primary care. It’s specialty care delivered without unnecessary barriers. It’s happening. We’re starting the year this way. And I’m going to keep pushing the boundaries and keeping you informed.

There's a pattern playing out in academic medical centers. Cardiac surgery isn't being called as a therapy. It's being called as a last resort. A patient comes in with endocarditis. DNR. DNI. Medical service admits them. A week goes by. They throw an embolus. The toe turns blue. The infection isn't clearing. And then someone says: "If you don't have an operation, you're going to die." And now the patient says yes. "You've been in the hospital for 10 days, you were DNR DNI, and now you're going to have a double valve with a reconstruction of the aortomitral curtain." Wait a second. This isn't a heart team decision. This is what happens when surgery is treated as a disposition for the sickest, most terminal patients — instead of a viable therapy offered early enough to actually matter.
