Contrast modulation is also helpful for microchannel tracking RCA & LAD CTO. RCA has a microchannel but wasn't able to cross with multiple wires. Did Contrast modulation then it was crossed easily with PJ wire without tip bend. LAD was crossed with Gaia 3rd. Timi III flow.

Great case Imran. 👏🏼 Did you use contrast (modified Carlino) for both vessels? If so, did you save the injection images? Re: the RCA - I think contrast can facilitate crossing of established microchannels. Wonderful results. 👌🏼

Yes in both vessels. But unfortunately not saved for LAD. You are right contrast facilitates the wire crossing in the establishment microchannel. Maybe these are interrupted microchannels. You are being investigator and the author can elaborate on it better.

With guide only, its more luck, as you may raise a flap proximally. . Probaby better with microcatheter in lesion.

A 4-year-old boy was at a barbecue eating when he suddenly grabbed his ear and started crying in pain. His ear exam was completely normal, so doctors sent him home with ibuprofen and numbing drops. Over the next several days he kept having ear pain, then developed fevers and stopped eating and drinking. Multiple exams still looked normal… until they did a CT scan with contrast. They found a metal wire from a grill brush lodged in the tissues near his tonsil. It had broken off into the hamburger he was eating and gotten stuck. The ear pain was actually referred pain from the wire irritating the area near his eustachian tube. He ended up needing surgery to remove it. The doctor’s warning at the end is important: Don’t use grill brushes with metal wires. Those little wires can break off into the food and cause serious problems — not just in the throat, but also in the stomach and intestines if swallowed. I had no idea those wires could break off like that — I’d never even thought about it. I’m definitely never using a metal grill brush again. Have you ever heard of grill brush wires causing problems like this? Or do you already avoid the metal ones?

Recently did some gun "spring cleaning." Sold off 5 guns I didn't like or didn't end up using much. Among these was my Glock 19, which I was never any good with no matter how much I tried. I also didn't like that as a Gen 3 Glock, it was not lefthand friendly. Plus, with the looming sales ban in our dumb state, now was a great time to sell, prices are high. But I still have a bunch of mags and holsters, so I decided to replace it with a Shadow Systems MR920, which at least allows for a lefthand mag release. Today, I zeroed its dot from a rest, then recorded this video. I *instantly* like this gun more than I ever did the G19. While it has a number of improvements (backstraps let me have a more 1911-ish grip angle, gas pedal, etc.), I think the main reason it clicked with me was the trigger. It's just lighter than the G19's was. That gun's "wall" never worked for me, I sucked with it. The MR920, by contrast, feels extremely similar to my P320 triggers, which I love. So, Shadow Systems W. But I'm also hyped for the other gun I'm picking up soon, a Staccato HD P4. Which will be by far my costliest gun 🤣. I didn't think I'd ever drop that kind of cheddar, but it never had occurred to me to pay for it with existing guns that were gathering dust in the safe.

This was the hardest lift I've ever done a 720 Yoke walk. To put in perspective... someone who is good at this lift would complete the movement in 11-15 seconds it took me dang near a min! Remember it like it was yesterday... I was battling out for a top 3 spot with some great lifters. The guy who was placed ahead of me was way better at this event then me. He RAN with the weight... but dropped it once... (not a big deal... you get 2 drops...) Picked it up quick and stumbled and dropped again! He was trying to catch the the second place spot. I knew this was my worst event and wasn't going to win it. BUT... if I made it down I would position myself in 3rd place going into a couple event I was good at. I struggled..... getting this thing down there. But did. It was the most proud I even been doing a lift. Even more then squatting 700. This took everything in me! Ended up placing 3rd in the event largely due to not giving up on this lift.

Together with UC Berkeley we are announcing the laser phase plate - a breakthrough in atomic resolution imaging. This is the brightest continuous wave laser in the world, 100 million times the intensity of the surface of the sun. Phase contrast plays an important role in microscopy, but it was thought close to impossible for electron microscopy, where it would require interfering with an electron beam. Holger Mueller and Robert Glaeser proposed exactly this using a standing wave laser. It has taken over 15 years to make this a reality. Biohub partnered with UC Berkeley and Mueller to support this work and to engineer and build the technology. Contrast has been the critical barrier to achieving atomic resolution imaging of the cell. In cryo-electron tomography, a cellular imaging technology that uses electron microscopy, the low contrast makes it impossible to resolve anything but the largest proteins within their cellular context. The laser phase plate removes that barrier. With advances in AI this breakthrough in contrast will start to open up a new frontier in structural biology, that will allow us to see the molecular machines of the cell, and how they assemble into far more complex and dynamic systems, and understand how they work.
