An 80-year-old man with high-risk, locally advanced prostate cancer (PSA 89, GG4, MRI PI-RADS 5, PSMA PET/CT with bilateral seminal vesicle invasion and pelvic nodal disease) presented with urinary retention. After initiating androgen deprivation therapy, we performed HoLEP before definitive radiotherapy. Despite the advanced disease, the procedure was remarkably straightforward: minimal bleeding, excellent surgical planes, no technical challenges beyond standard HoLEP, and successful spontaneous voiding just 36 hours after catheter removal. This case highlights that HoLEP before radiotherapy is feasible, safe, and should not be avoided solely because of locally advanced prostate cancer. #HoLEP #ProstateCancer #Urology #Endourology #RadOnc

I'm sharing a #RARP after a #HOLEP. The patient I operated on two years ago was completely asymptomatic, however, he presented with an elevated PSA of 2.4 ng/ml, when he had previously managed below 1 ng/ml. I ordered an MRI and PET scan PSMA, followed the protocol, and performed the radical prostatectomy. The enucleation appears to have been perfect, and it's interesting to see the completely clear prostatic bed inside. Do you routinely follow up your Holep patients even if they are asymptomatic?

This was my first HoLEP with an indurated or stony prostate (fixed to the rectum) due to Cancer Gleason 4+3 and PSA of 28. Patient in urinary retention with an indwelling catheter, to be honest, I was not sure that he had dissection planes, so we prepared the bipolar for TURP, to my pleasant surprise it was easier than expected, with some moments of small bleeding. Based on available information, Biden’s cancer is likely to have formed sometime after 2019 and may have been missed because of a false negative from medication he was taking — he probably didn’t have bone metastases until 2024 or early 2025. Here’s why ⬇️ In 2019 Biden’s physician said Biden is being treated for BPH (benign prostatic hyperplasia). He probably had a raised PSA at the time. Did they do an MRI of the prostate to check for lesions? I would guess yes, and they would have seen any lesions/nodules at this time. This was the only time he mentioned Biden’s prostate, he specifically said no prostate cancer and he didn’t mention which medication the BPH was being treated with. Feb 2023, it was mentioned Biden has “normal wear and tear of the spine” and while he didn’t specify which imaging modality was used, he mentioned “no nerve root compression” indicating a spinal MRI. If there were spinal bone metastases at this time, they would have likely seen them. Feb 2024, it stated “unchanged spinal arthritis without compression of nerve root.” They also mention hip arthritis. Maybe another MRI, spine and hip? If so, any bone lesions? July 2024, Biden gets Covid. Does he get a CT Chest? Do they see any bone lesions indicating metastatic disease? A few days later he dropped out of the presidential race. Is this why? Biden’s spokesperson says his last PSA test was 2014, which I find hard to believe if they were still treating him for BPH in 2019. They specifically said he didn’t have prostate cancer in 2019, how would they know if they didn’t check PSA or do imaging? How were they monitoring it? Symptoms only? Was Biden actually getting PSA tests to monitor his BPH and screen for prostate cancer? He was getting skin checks and colonoscopies so he was still being screened for common cancers despite his age. Was he taking Finasteride for the BPH (Trump has also taken this medication in the past) which may lead to a false negative PSA, ultimately masking the cancer causing it to be missed? I am humbled to share a significant milestone in pancreatic surgery: I recently performed the first-ever da Vinci single-port robotic distal pancreatectomy and splenectomy without an additional incision for pancreatic cancer in the United States, approved by both the Institutional Review Board and IDE from the FDA through a 2cm incision inside the belly button leaving no additional scar. This pioneering procedure not only achieved negative margins but also allowed for the thorough harvesting of all local lymphatics, a critical factor in ensuring comprehensive cancer treatment. I am grateful to report that the patient is doing exceptionally well, and it was a privilege to see him in the clinic several days post-operation. The images included illustrate the operating room moments and a snapshot of our follow-up clinic visit with my remarkable ARNP Dana Manzi and myself.
