We recently performed a successful #TAVR in a frail 79-year-old lady weighing just 35kg, with severe aortic valve stenosis with severe left ventricular dysfunction (EF 20%) at Kauvery Hospital, Marathahalli. She did develop some rhythm issues and also had to undergo a permanent pacemaker implantation after a day. We were able to discharge her after 4 days. Prior to this, she was suffering from frequent breathing issues and heart failure, leading to multiple hospitalizations. #medtwitter TAVR (transcatheter aortic valve replacement) is an effective alternative to patients who can not undergo open heart valve replacement due to high surgical risk.

#TAVR Bumper Technique This is something I’ve been doing and it makes crossing the aortic valve easy without pre BAV. After final valve assembly in descending aorta, roll contrast into the nose cone to create the bumper. 1. Reduces scraping in aorta. 2. Crosses like butter!

Device embolization is a rare but potentially life-threatening complication of #TAVR procedures. In this #JACCINT case, the very dilated aortic root permitted valve inversion, which was successfully conservatively managed. Learn more:

Nice teaching TAVR. Degenerated 23mm CE valve. Predominant AI. Should be an easy case?! Very difficult to cross with valve even after upgrade to Lunderquist. Attempts to deploy valve resulted in exaggerated LV displacement despite pulling valve and pushing wire.

We’ll all be seeing more of this. TAVR-in-TAVR due to calcific degeneration. A bit of nuance to decide on valve sizing. 26+3 resilia in a 29 S3. MG 45-> 4 mmHg. MCG Cardiology Fellowship

AVR with Chitra valve one year back, CAG then was normal. Irregular with OAC & INR monitoring. INR on admission was 1.3, no visible thrombus on aortic prosthesis. Admitted with STEMI AW. How to manage? Advice please
