The first time I did brain surgery alone from start to finish during my training shouldn’t have happened. A woman was crashing from a massive brain bleed, and the attending on call never answered my 14 calls or came in. With her life at risk, I chose to operate alone rather than keep waiting. What would you have done? The patient recovered well. Before starting, I called a mentor I trusted, explained the situation, and got his blessing to proceed, so I broke no rules. Still, performing the entire operation without an attending at least showing up in the OR was completely new at my program and at my level of training. I am still bothered 25 years later: a surgeon routinely operated 72 hours without sleep doing ~14 surgeries, some nonurgent. He hunted cases, told elective patients surgery was “urgent,” then vanished; trainees managed complications. I reported him. Got nowhere. How would you stop him? #PatientSafety
