Chest tube insertion is a fundamental skill. Here, we review standard tube thoracostomy: ✔️ For traumatic hemo/pneumothorax, small bore chest tubes are non-inferior to large bore chest tubes ➡️28 Fr is the largest tube you need. ✔️ Insert at intersection of anterior axillary line & nipple/inframammary fold ➡️ ~5th intercostal space. ✔️When time allows, anesthetize the site of insertion, including the skin, subcutaneous tract, intercostal muscle, & periosteum. 🧵 1/2

If you don’t mind a suggestion: 1) I recommend an intercostal nerve block once the intercostals are exposed during in order to avoid procedural pain in patients; 2) I recommend spreading, not popping, through the intercostal muscles, which is more comfortable for awake pts.

What is the male "monkey dance?" In his book Meditations on Violence, law enforcement consultant Rory Miller describes "the monkey dance" as a ritualized form of male combat to establish dominance or secure territory. REVIEW: How Minnesota House DFL is gaslighting the public and spinning the narrative and the #MNmedia propaganda machine are just repeating the Dems talking points. 1- MN House Dems are boycotting because they did NOT want MN House Republicans to elect Lisa Demuth the Speaker (we did anyways) and NOT because of Tabke race (which is their spin), but because we had the votes and constitutional authority to elect her. 2- They continue to say “we’re tied”….NO we’re not! 67 (R) to 66 (D) is clearly a Republican Party of Minnesota majority in the House 3- WHY are they at 66? Their 40B candidate cheated on residency and does not live in the District that he ran for office (they keep skipping over this and hope MN is so stupid that you don’t know the truth…..Cheating has consequences and they like children, want to skip these consequences.) 4- 54A race in City of Shakopee, MN has had multiple procedural errors. Confidence is shaken in this District that their votes actually counted. ✔️ Race originally had Aaron Paul winning with 100% precincts reporting by 360 votes ✔️ 20 votes shredded ✔️ 30 Duplicate voters ✔️ “Official data” at SOS office continued to change “after” Dec 17th deadline that Counties were supposed to be done uploading. (We have all this documented.) ✔️ At trial, Scott County MN employee testified that they could NOT guarantee voters who testified at trial that it was 100% for sure their ballot which was shredded 5- There was a trial in 54A. The judge ruled AND Aaron Paul has every right to appeal this ruling. It would be honorable if Melissa Hortman would STOP gaslighting the public and ignoring the legal process has NOT been completed. 6- “If” there was a new election in 54A ……zero people are disenfranchised because all 22,000 are free to vote again. The Minnesota DFL Party continues to deflect, lie, deceive, manipulate the public and put out these false narratives because they hope the public is so naive that you don’t do your own research and instead just believe all of their fear-mongering and purposeful #gaslighting! In My Opinion: The Democrats are the ones disenfranchising our MN voters, they are deteriorating the public’s trust in our election process by trying to gaslight all of you into believing this is settled. So they have this illusion that they have all these choices. “But the fact of the matter is, if you want to be good you don’t really have a lot of choices. It takes what it takes. You have to do what you have to do to be successful. “You have to make the choices and decisions to have the discipline and the focus to the process of what you need to do to accomplish your goals. “All these guys that think they have a lot of choices are sadly mistaken. As we all have done with our own children, they learn these lessons of life as they get older. “Sometimes the best way to learn is from the mistakes you make, even though we all hate to see them have to make them, and we don’t condone it when they do.” – I’ve studied Saban for 12+ years, and the Illusion of Choice is one of the most powerful concepts I’ve come across. Thoracic and Abdominal Cavities Subcutaneous hemorrhage in the chest wall. Rib fractures: - Left: ribs 3-5 - Right: ribs 3-6 Intercostal muscles show hemorrhage at fracture sites. Sternum shows corresponding injury, likely from strong frontal impact. Diaphragm shows signs of damage; possible emergency thoracotomy. 𝗖𝗮𝘀𝗲 𝗦𝘂𝗺𝗺𝗮𝗿𝘆 The Tuanjie Police Station of Chaoyang District, Beijing, commissioned the Beijing Shengtang Forensic Identification Institute to conduct an autopsy and determine the cause of death of a 37-year-old male surnamed Yu. 𝗜𝗜𝗜. 𝗔𝘂𝘁𝗼𝗽𝘀𝘆 𝗙𝗶𝗻𝗱𝗶𝗻𝗴𝘀 1. External Examination - Height: 181 cm - Body build: Normal development, average nutrition, pale yellow skin. - Superficial lymph nodes: No palpable enlargement. - Postmortem lividity: Dark red, concentrated in the occipital, parietal, back, lumbar-sacral regions and lower extremities. - Areas not under pressure: Color unchanged upon compression. - Abdomen: Early greenish discoloration indicating initial decomposition. 2. Head and Face - Hair black, thick, approximately 12 cm long. - Scalp shows contusions and lacerations (details obscured). - Eyelids and conjunctiva pale; corneas cloudy, pupils non-reactive to light. - Traces of blood in both ears, external auditory canals, and nose. - Irregular wound approximately 3 cm long, corresponding irregular horizontal wound of about 7.5 cm on the outer surface. 3. Neck - Bruising on the right side of the neck; subcutaneous tissue and neck muscles show hemorrhage. - Soft tissue and muscles in the neck area exhibit marked subcutaneous bleeding. - Hyoid bone and thyroid cartilage show compression , suggestive of manual strangulation or ligature strangulation. 4. Oral Cavity - All teeth missing except for the lower right first premolar. - Oral mucosa and gums show necrotic changes. 5. Trunk and Limbs - Sutured wound on the left flank and upper left abdomen. - Right diaphragm appears distended. - Old scars on the backs of both hands. - No fractures detected in limbs, joints relatively intact. 6. Genital and Anal Area - External genitalia normal. - Anus shows a fresh circular wound about 5 cm in diameter. - Internal and external sphincter muscles severely torn. - Evidence of hemorrhage around the area. 𝗜𝗩. 𝗜𝗻𝘁𝗲𝗿𝗻𝗮𝗹 𝗘𝘅𝗮𝗺𝗶𝗻𝗮𝘁𝗶𝗼𝗻 1. Head - Scalp hematoma present. Galea aponeurotica shows hemorrhage. Anterior and posterior fontanelle gaps abnormally widened (not typical in adults). - Subdural space shows hemorrhage. Indicates closed cranial trauma. 2. Thoracic and Abdominal Cavities Subcutaneous hemorrhage in the chest wall. Rib fractures: - Left: ribs 3-5 - Right: ribs 3-6 Intercostal muscles show hemorrhage at fracture sites. Sternum shows corresponding injury, likely from strong frontal impact. Diaphragm shows signs of damage; possible emergency thoracotomy. Pleural fluid: - Left: approx. 150 ml - Right: approx. 200 ml Pericardial cavity: approx. 160 ml of bloody fluid. Abdominal cavity: Abdominal wall fat thickness: 0.5 cm - Upper abdomen with a hemorrhagic region measuring about 16.5 × 7 cm - Small intestine crushed, mucosa congested and reddened - About 860 ml of clear yellow fluid in the abdominal cavity. 3. I’m a woman of strength, determination and courage. If you don’t like me, that’s cool, but I know in my heart there are some great accounts here that have kept X moving and I don’t think X wants them to walk away!
