I don't know much about rotary wing but isn't the pilot supposed to fly inside the helicopter?

Possible scenario: The pilot left the cockpit with rpm at flight rpm without friction on the collective. Collective jumped up and made the skids light, without pedal input to counter torque, it started spinning.

What happened? Did the collective vibrate up a bit or was it not all the way down and when they got out there was sufficient lift available to do that?
